Abstract

Stroke merupakan penyebab kematian dengan prevalensi yang tinggi secara gobal. Pasien dengan stroke cenderung mengalami penurunan fungsi dan perubahan peran fisik, gangguan kognitif serta mengalami gangguan mood dan sosial sehingga diperlukan perawatan yang berkesinambungan. Peningkatan kualitas hidup adalah tujuan utama yang diharapkan dalam perawatan pasien stroke pasca rawat inap.Tujuan dari penelitian ini adalah untuk menganalisa pelaksanaan discharge planning pada pasien stroke di rumah sakit. Metode penelitian yang digunakan adalah analisa tematik, metode pendekatan sederhana (simplified approach method). Pengumpulan data dilakukan sejak april sampai juli tahun 2018 dan pencarian menggunakan database Google Cendekia, EBSCO, Pubmed, IndonesiaOneSearch dan Proquest. Hasil: Berdasarkan hasil dari ketujuh artikel yang telah dikaji didapatkan tiga tema terkait pelaksanaan discharge planning pada pasien stroke yaitu peran perawat, manfaat dan metode. Kesimpulan: Peran perawat merupakan salah satu kunci penting dalam pelaksanaan discharge planning, terdapat manfaat yang dirasakan oleh pasien dan keluarga dari segi pengetahuan, fisik dan psikososial serta terdapat beberapa metode dalam pelaksanaan discharge planning yang dapat diimplementasikan. Saran: peneliti menyarankan agar mengadakan penelitian yang lebih lanjut mengenai pelaksanaan discharge planning pasien stroke, dapat menjadi referensi ilmu pengetahuan dalam pemberian asuhan keperawatan pada pasien stroke, serta kepada masyarakat khususnya keluarga dari pasien stroke agar menjadi support system yang baik = Stroke is a cause of death with a high prevalence globally. Patients with stroke tend to experience decreased function and changes in physical role, cognitive impairment and experiencing mood and social disorders so that continuous care is needed. Improvement of quality of life is the main goal expected in the treatment of post-hospital stroke patients.The purpose of this study is to analyze the implementation of discharge planning in stroke patients in the hospital. The research method applies is thematic analysis, the simplified approach method. Data collection was conducted from April through July 2018 and using database Google Scholar, EBSCO, Pubmed, IndonesiaOneSearch and Proquest. Results: Based on the results of the seven articles that have been studied, there are three themes related to the implementation of discharge planning in stroke patients, there are role of nurses, benefits and methods. Conclusion: The role of nurses is important key in the implementation of discharge planning, there are benefits felt by patients and families in terms of knowledge, physical and psychosocial and there are several methods in implementing discharge planning that can be implemented. Suggestion: The researcher suggests that to conduct further research on discharge planning of stroke patients, it can be a reference of education in provision of nursing care to stroke patient, and to society especially family from stroke patients in order to become a good support system.
