Abstract

Background:The phenomenon are about the rehospitalization increased of Chronic Kidney Disease patients, seen from the CKD patients amount in the three last monts asa many as 824 patients, and then less education about fluid restriction, proper nutrition recommendations, family education for prepare to discharge by nurse, and ineffective discharge planning because patients not cooperative in Hemodialysis control education, patients unfollowing the control schedule and patient can’t speak in Indonesia so or can’t understand what nurses said Aim: to describe the factors influencing the discharge planning by nurses for CKD patients in Siloam Hospitals Makassar care rom. Method: using descriptive quantitative method with cross sectional appoach. The study population od all nurses im SHMK care room. Sampling method using purposive sampling technique.. The data were collected using questionnaire and data analysis using univariate data analysis Result : it shows that the personnel factor have good category is 54%, the involvment and participation factor have less category is 54%, the time factor have good category is 54%, communication factor have good category is 57% and the agreement and consent are 52%. It is expected that further research can be done by observation to see the description of the factors that influence the discharge planning implementation by the nurse for CKD patients / Latar belakang : fenomena yang ditemukan adalah tingginya angka rehospitalisasi pasien dengan Chronic Kidney Disease, yang dilihat dari tingginya angka pasien CKD dalam 3 bulan terakhir yaitu sebanyak 824 pasien, kemudian perawat kurang mengkomunikasikan akan apa yang harus dipatuhi oleh pasien, seperti batasan cairan, anjuran nutrisi yang tepat, edukasi keluarga selama perawatan untuk persiapan perawatan dirumah, dan pelaksanaan discharge planning yang kurang efektif, karena pasien yang kurang kooperatif dalam edukasi kontrol Hemodialisa, pasien yang tidak mengikuti jadwal kontrol, tidak dapat berbahasa Indonesia sehingga kesulitan menerima edukasi. Tujuan : Untuk memberi gambaran faktor – faktor yang memengaruhi pelaksanaan discharge planning oleh perawat terhadap pasien CKD di ruang perawatan Siloam Hospitals Makassar. Metode : kuantitatif deskriptif dengan pendekatan cross sectional. Populasi penelitian semua perawat di Siloam Hospitals Makassar, pengambilan sampel dengan purposive sampling. Pengumpulan data penelitian menggunakan kuesioner dan analisa data univariat. Hasil Penelitian Menunjukan bahwa faktor personil dengan kategori baik sebanyak 54%, keterlibatan dan partisipasi kategori kurang sebanyak 54%, faktor waktu dengan kategori baik sebanyak 54%, faktor komunikasi sebanyak 57% dan perjanjian dan konsensus dengan kategori kurang sebanyak 52%. Diharapkan untuk penelitian selanjutnya dapat meneliti dengan melakukan observasi untuk melihat gambaran faktor-faktor yang memengaruhi pelaksanaan discharge planning oleh perawat pada pasien CKD.
