Abstract

Medication Error is any avoidable occurrence that causes or results in inappropriate drug service or harm to the patient while the drug is under the supervision of a health or patient (NCC MERP, 2012). According to Rusmi (2012) Medication error occurs in the treatment process such as prescribing, transcribing, dispensing and administration. Based on Ministry of Health, (2008) Error in drug delivery was ranked first (24.8%) of the 10 reported incidents. The types of errors that cause deaths in patients include 40.9% false doses, 16% false drugs, and 9.5% wrong route of administration. The purpose of this study to determine the perception of nurses to medication administration errors that occur. This research is a quantitative research with descriptive design. The population of this research is all nurses of department Haemodialisis, Chemotherapy, and ICU that is as many as 34 people. This research uses total sampling technique with nurse perception research about medication administration error is positive perception 17 people (50%) and negative perception 17 people (50%). The results show the majority of nurses performing a re-checking of the patient's identity while giving the drug, while a small nurse checks the room number or patient's location as the patient's identity. Suggestions: Hospitals can provide training, education, motivation, and evaluation of medication errors to nurses / Medication Error adalah setiap kejadian yang dapat dihindari yangmenyebabkan atau berakibat pada pelayanan obat yang tidak tepat atau membahayakan pasien sementara obat berada dalam pengawasan tenaga kesehatan atau pasien (NCC MERP, 2012). Menurut Rusmi (2012) Medication error terjadi pada proses pengobatan antara lain prescribing (peresepan), transcribing (penerjemahan resep), dispensing (penyiapan) dan administration. Berdasarkan Kemenkes, (2008) Kesalahan dalam pemberian obat menduduki peringkat pertama (24,8%) dari 10 besar insiden yang dilaporkan. Tipe kesalahan yang menyebabkan kematian pada pasien meliputi 40,9% salah dosis, 16% salah obat, dan 9,5% salah rute pemberian. Tujuan penelitian ini untuk mengetahui persepsi perawat terhadap medication administration errors yang terjadi. Penelitian ini merupakan penelitian kuantitatif dengan desain deskriptif. Populasi penelitian ini adalah seluruh perawat departemen Haemodialisa, Chemotherapy, dan ICU yaitu sebanyak 34 orang. Penelitian ini menggunakan teknik pengambilan sampel total sampling dengan hasil penelitian persepsi perawat mengenai medication administration error adalah persepsi positif 17 orang (50%) dan persepsi negatif 17 orang (50%). Hasil menunjukan mayoritas perawat melakukan tindakan pengecekan kembali identitas pasien saat akan memberikan obat, sedangkan sedikit perawat yang melakukan tindakan pengecekan nomor kamar atau lokasi pasien sebagai identitas pasien. Saran: Rumah Sakit dapat memberikan pelatihan, edukasi, motivasi, dan evaluasi mengenai medication administration error kepada perawat.
