Abstract

The incidence of cardiac arrest in Indonesia is approximately ten out of 100,000 normal people under the age of 35 and reaches about 300,000-350,000 incidents annually. Delay in knowing or coping with changes in the physiological parameters will increase the risk of death in hospital and sudden cardiac arrest events that often cause emergency calls or Code Blue. Early Warning Score is an early detection of worsening the condition of the patient by using a reference of vital signs or physiological parameters that may affect the number of occurrences of code blue in the hospital. The Purpose of the reseacrh is to find out the complete picture of the documentation of Early Warning Score (EWS) at Pavilion Siloam General Hospital. The method used is documentation study with quantitative descriptive method. Data collection of 25 Code Blue patient files retrieved from January to June 2017. The results showed that from 25 observed files, the accuracy of the assessment was 74.6%, 100% documentation update, and document completeness of 78.6%. From the results of research completeness of the documentation Early Warning Score overall get a value of 28% or can be said low / Angka kejadian henti jantung di Indonesia kira-kira sepuluh dari 100.000 orang normal yang berusia dibawah 35 tahun dan mencapai sekitar 300.000- 350.000 kejadian setiap tahunnya. Keterlambatan dalam mengetahui maupun mengatasi perubahan parameter fisologis akan meningkatkan resiko kematian di rumah sakit dan kejadian henti jantung secara mendadak yang sering menimbulkan panggilan darurat atau Code Blue. Early Warning Score merupakan sebuah metode deteksi dini perburukan kondisi pasien dengan menggunakan acuan tanda-tanda vital atau parameter fisiologis yang dapat berpengaruh terhadap angka kejadian code blue di rumah sakit. Tujuan penelitian ini adala untuk mengetahui gambaran kelengkapan dokumentasi Early Warning Score (EWS) terhadap kejadian code blue di Paviliun Rumah Sakit Umum Siloam. Metode yang digunakan adalah studi dokumentasi dengan metode deskriptif kuantitatif. Pengambilan data terhadap 25 file pasien Code Blue yang diambil dari bulan Januari sampai Juni 2017.Hasil penelitian didapatkan bahwa dari 25 file yang dilakukan observasi, ketepatan penilaian 74,6%, kebaharuan dokumentasi 100%, dan kelengkapan dokumen 78,6%. Dari hasil penelitian kelengkapan dokumentasi Early Warning Score secara keseluruhan mendapatkan nilai 28% atau dapat dikatakan rendah.
