Abstract

Latar Belakang : Diabetes Mellitus (DM) tipe II saat ini menjadi salah satu keprihatinan dunia dan menjadi penyebab dari morbiditas dan mortalitas dini. Secara global, sekitar 1 dari 11 orang dewasa mengidap DM, dan Indonesia menduduki peringkat ke-6 untuk jumlah penderita DM terbanyak sedunia. Sedangkan untuk jumlah prevalensi DM di Indonesia, Provinsi Sulawesi Utara menempati peringkat ke-4. DM tipe II dipengaruhi oleh komponen genetik dan lingkungan dengan sama kuatnya yang terlihat jelas dengan besarnya jumlah penderita DM tipe II yang memiliki riwayat penyakit ini di dalam keluarganya. Riwayat keluarga merupakan salah satu faktor risiko DM tipe II yang tidak dapat dimodifikasi dan dapat mempengaruhi onset usia penyakit ini. Hal ini diduga karena peran dari gabungan gen pembawa sifat DM dari ayah dan ibu sekaligus dapat mempercepat onset perkembangan DM tipe II. Studi-studi sebelumnya menunjukkan bahwa individu dengan riwayat keluarga DM tipe II memiliki onset usia yang lebih muda bila dibandingkan dengan individu tanpa riwayat keluarga DM tipe II. Tujuan Penelitian : Mengetahui perbedaan onset usia saat terdiagnosis DM tipe II pada subjek dengan dan tanpa riwayat keluarga DM tipe II di Manado. Metodologi Penelitian : Penelitian ini menggunakan desain studi cross-sectional pada 142 pasien DM tipe II di Manado. Pengambilan data menggunakan metode Purposive Sampling dan dikumpulkan melalui media Google Form. Responden dikelompokkan berdasarkan onset usia saat terdiagnosis DM tipe II menjadi dua kelompok, yaitu ≥45 tahun dan <45 tahun, hasil kemudian dianalisa dengan uji Chi-square dan regresi logistik. Hasil : Dari 142 responden, dengan menggunakan uji bivariat Chi-square ditemukan onset usia saat terdiagnosis DM tipe II memiliki hubungan yang bermakna dengan riwayat keluarga DM tipe II (p = <0,001; OR = 8,357). Sisi riwayat keluarga DM tipe II juga ditemukan memiliki hubungan yang bermakna dengan onset usia saat terdiagnosis DM tipe II (p = <0,001). Riwayat keluarga DM tipe II dari sisi ayah dan ibu sekaligus memiliki risiko tertinggi untuk terdiagnosis dengan DM tipe II pada usia yang lebih muda (OR = 16,333), diikuti oleh kelompok responden dengan riwayat keluarga DM tipe II dari sisi ayah (OR = 8,167), dan dari sisi ibu (OR = 6,91). Kesimpulan : Terdapat perbedaan yang signifikan pada onset usia saat terdiagnosis DM tipe II pada subjek dengan dan tanpa riwayat keluarga DM tipe II, dimana subjek dengan riwayat keluarga DM tipe II memiliki peluang lebih tinggi untuk terdiagnosis dengan DM tipe II pada usia yang lebih dini. Kelompok subjek dengan riwayat keluarga DM tipe II dari sisi ayah dan ibu sekaligus memiliki risiko tertinggi untuk terdiagnosis dengan DM tipe II pada usia yang lebih dini. Kata Kunci : Onset Usia Diagnosis, Diabetes Mellitus (DM) Tipe II, Riwayat Keluarga / Background : Type II Diabetes Mellitus (T2DM) nowadays have become one of the world’s source of concern and also the cause of early morbidity and mortality. Globally, around 1 out of 11 adults have diabetes, and Indonesia is in the 6th rank for the highest number of diabetes patients in the world. For the number of diabetes prevalence in Indonesia, North Sulawesi Province was ranked 4th. The development of T2DM is strongly influenced by genetical factor and environmental factor equally, which is seen by the large number of T2DM patients with T2DM history in their family. Family history is one of the risk factor for T2DM which cannot be modified and can influence the age of onset of this disease in individuals with T2DM family history. This is presumably because the role of diabetic carrier genes combination from patient’s father and mother can accelerate the development onset of T2DM. Studies have shown that there is a difference in the T2DM age of onset, where the T2DM age of onset in individuals with T2DM family history is younger than in individuals without T2DM family history. Aim : To determine the difference in onset age at diagnosis of T2DM between subjects with and without family history of T2DM in Manado. Methods : This study was conducted with cross-sectional study design on 142 T2DM patients in Manado. Samples were collected with purposive sampling technique and through Google form. Based on the onset age at diagnosis of T2DM, samples are grouped into two groups, which are ≥45 years old and <45 years old, results were then analysed using Chi-square and Logistic Regression analytic methods. Results : From 142 respondents, bivariate analysis of Chi-square shows that onset age at diagnosis of T2DM had significant relationship with family history of T2DM (p = <0,001; OR = 8,357). Which side of the family with T2DM history also had significant relationship with onset age at diagnosis of T2DM (p = <0,001). Family history of T2DM from both father and mother shown to have the highest risk of early onset age at diagnosis of T2DM (OR = 16,333), which then followed by group of subjects with T2DM family history from father (OR = 8,167), then mother (OR = 6,91). Conclusion : There are significant difference in onset age at diagnosis of T2DM between subjects with and without family history of T2DM, where subjects with family history of T2DM have higher risk for early onset age at diagnosis of T2DM. Group of subjects with T2DM family history from both father and mother have the highest risk of early onset age at diagnosis of T2DM.
