Abstract

Latar belakang: Kejadian stroke yang didominasi oleh stroke iskemik dapat mengakibatkan defisit neurologi persisten yang berdampak pada produktivitas pasien. Stroke juga berkaitan dengan angka kematian yang tinggi dan efek negatif terhadap ekonomi masyarakat. Beberapa parameter fisiologis seperti osmolaritas plasma telah dikaitkan dengan derajat keparahan stroke yang diukur dengan skor NIHSS. Beberapa penelitian sudah dilakukan sebelumnya namun masih menunjukan ketidaksesuaian hasil sehingga perlu dilakukan penelitan kembali mengenai hubungan osmolaritas tinggi dengan skor NIHSS pada pasien stroke iskemik akut. Tujuan: Penelitian ini bertujuan untuk melihat hubungan antara kenaikan osmolaritas plasma dengan skor NIHSS pada pasien stroke iskemik akut di Siloam Hospital Lippo Village Gedung B. Metode: Penelitian ini menggunakan studi analitik dengan desain potong- lintang. Pengambilan data akan dilakukan menggunakan data sekunder dari 66 rekam medik pasien yang menderita stroke iskemik akut pertama kali di Siloam Hospital Lippo Village Gedung B. Analisis data akan menggunakan analisis korelasi dengan program SPSS 27.Hasil: Skor NIHSS pasien stroke iskemik akut saat admisi berhubungan signifikan dengan osmolaritas plasma yang menunjukan arah korelasi positif dengan kekuatan korelasi sedang (p= 0.004, r= 0.309). Didapatkan skor NIHSS lebih tinggi pada pasien stroke iskemik akut dengan kenaikan osmolaritas plasma dibandingkan tanpa kenaikan osmolaritas plasma (8.52 ± 5.01 vs 6.03 ± 3.72).Faktor resiko jenis kelamin laki-laki (p= 0.002) dan dislipidemia (p= 0.028) berpengaruh terhadap skor NIHSS pasien stroke iskemik akut dengan dan tanpa kenaikan osmoalritas plasma. Kesimpulan: Kadar osmolaritas plasma yang lebih tinggi pada pasien stroke iskemik akut berkorelasi dengan kenaikan skor NIHSS saat admisi./ Background: The incidence of stroke which is dominated by ischemic stroke can result in persistent neurological deficits that impact patient productivity. Stroke is also associated with high mortality rate and have a negative effect on the economy. Several physiological parameters such as plasma osmolarity have been associated with stroke severity as measured by the NIHSS score. Studies have been conducted previously but still show discrepancies in the results, therefore, it is necessary to re-examine the relationship of high plasma osmolarity with NIHSS score in acute ischemic stroke patients.Aim: The aim of this study is to understand the relationship between high plasma osmolarity with NIHSS score in patients diagnosed with acute ischemic stroke in Siloam Lippo Karawaci General Hospital.Methodology: This study is an analytical study with a cross-sectional study design. Data will be collected using secondary data from 66 medical records of patients diagnosed with acute ischemic stroke for the first time at Siloam Lippo Karawaci General Hospital. Data will be analysed by correlation analysis test using SPSS 27.Result: NIHSS score at admission in patients with acute ischemic stroke was significantly associated with plasma osmolarity that showed a positive correlation with moderate strength (p= 0.004, r=0.309). NIHSS score was found to be higher in acute ischemic stroke patients with increased of plasma osmolality compared to patients without an increase in plasma osmolarity (8.52 ± 5.01 vs 6.03 ± 3.72). The risk factors of stroke such as male (p=0.002) and dyslipidaemia (p=0.028) had an effect on NIHSS stroke of acute ischemic stroke patients with and without an increase in plasma osmolarity Conclusion: Higher plasma osmolarity level in acute ischemic stroke patients was correlated with an increase in NIHSS score at admission.
