Abstract

Medication error atau kesalahan pengobatan merupakan peristiwa penggunaan obat yang tidak tepat sehingga membahayakan pasien. Salah satu penyebab adalah obat LASA atau Look Alike Sound Alike. Pada e- catalogue BPJS 2020 untuk sediaan injeksi generik sebagian besar masuk ke dalam golongan obat LASA. Pada penelitian ini sediaan yang diambil berupa vial dan ampul. Metode penelitian menggunakan Failure Mode Effect Analysis atau FMEA dengan menentukan angka RPN atau Risk Priority Number yang merupakan perkalian antara angka kejadian (Occurance), angka kegagalan (Severity), dan angka deteksi (Detection). Selain itu penentuan jenis risiko menggunakan "Priority Number". Hasil penelitian menunjukkan nilai tertinggi RPN pada Look Alike (LA) yaitu 125, sedangkan nilai tertinggi pada Sound Alike (SA) mencapai 75. Jenis resiko pada penelitian adalah Medium Risk dengan presentase obat Look Alike (LA) mencapai 77,14% sedangkan pada Sound Alike (SA) mencapai 70,18% / Medication error is an event of inappropriate use of drugs that endangers the patient. One of the causes is the drug LASA or Look Alike Sound Alike. In the BPJS 2020 e- catalogue for generic injection preparations, most of them fall into the LASA drug class. In this study, the preparations taken were vials and ampoules. The research method uses Failure Mode Effect Analysis or FMEA by determining the RPN or Risk Priority Number which is the product of the incidence (Occurance), the failure rate (Severity), and the detection rate (Detection). In addition, the determination of the type of risk uses the "Priority Number". The results showed the highest value of RPN in Look Alike (LA) was 125, while the highest value in Sound Alike (SA) reached 75. The type of risk in this study was Medium Risk with the percentage of Look Alike (LA) drugs reaching 77.14% while in Sound Alike (SA) reached 70.18%.
