Abstract

Klinik Gigi merupakan sarana atau tempat yang dibangun bagi pasien untuk mendapatkan pengobatan dan perawatan yang terkait dengan gigi. Perancangan sistem informasi klinik gigi ini adalah untuk mempermudah pendataan dan penyimpanan rekam medis yang telah dilakukan oleh pasien agar mempermudah pasien apabila rekam medis yang dilakukan hilang. Rekam medis dalam kedokteran gigi merupakan dokumen penting dalam bidang pelayanan medis dikarenakan dokumen rekam medis berisi data-data mengenai kondisi gigi pasien. Dengan adanya rekam medis tersebut semua tindakan yang dilakukan oleh kedokteran gigi dapat dilakukan dengan benar. Pendataan rekam medis ini juga harus menyesuaikan atau memenuhi standar nasional rekam medis gigi yang ada terutama yang akan dibahas adalah pendataan rekam medis odontogram. Odontogram adalah data yang penting dalam proses identifikasi dan merupakan salah satu data yang terlampir dalam rekam medik gigi. Data odontogram memuat data tentang jumlah, susunan, bentuk, tambalan gigi dan sebagainya. Odontogram adalah suatu gambaran peta mengenai keadaan gigi dan rongga mulut serta merupakan dokumen dari rekam medik kedoktertan gigi. Tugas akhir ini juga berfokus pada pengembangan sistem dengan menggunakan metodologi iterative model dengan menggunakan bahasa pemrograman PHP. Pemodelan yang digunakan dalam membangun sistem ini adalah dengan menggunakan UML seperti use case diagram, table relationship diagram, activity diagram, dan class diagram. Hasil dari tugas akhir ini adalah website untuk memudahkan mendata rekam medis dan dengan menggunakan odontogram / Dental Clinic is a facility or place built for patients to get treatment and treatment related to teeth. The design of this dental clinic information system is to facilitate the collection and storage of medical records that have been carried out by patients to make it easier for patients if the medical records carried out are lost. Medical records in dentistry are important documents in the field of medical services because medical records documents contain data about the patient's dental condition. With the medical record, all actions performed by dentistry can be done correctly. This medical record collection must also adjust or meet the national standards of existing dental medical records, especially what will be discussed is the data collection of odontogram medical records. The odontogram is an important data in the identification process and is one of the data attached to the dental medical record. Odontogram data contains data on the number, arrangement, shape, dental fillings and so on. The odontogram is a map of the condition of the teeth and oral cavity and is a document from the medical record of dental medicine. This final project also focuses on system development using an iterative model methodology using the PHP programming language. The modeling used in building this system is to use UML such as use case diagrams, table relationship diagrams, activity diagrams, and class diagrams. The result of this final project is a website to make it easier to record medical records and by using an odontogram.
