Abstract

Interaksi obat merupakan suatu masalah yang berkaitan dengan terapi obat dan dapat mempengaruhi hasil pengobatan klinis pasien. Interaksi obat dapat terjadi karena adanya polifarmasi, pasien yang memiliki penyakit akut atau komorbid dan pasien yang mendapatkan obat yang lebih kuat dengan indeks terapi yang sempit. Penelitian ini bertujuan untuk mengetahui gambaran potensi interaksi obat antidiabetik oral, mengetahui gambaran kuantitas peresepan antidiabetik oral, serta mengetahui tingkat keparahan (severity level) potensi interaksi obat antidiabetik oral pada lembar resep pasien diabetes melitus tipe 2 dengan komorbid di Instalasi Farmasi Rawat Jalan periode Januari-Juni 2021. Penelitian ini dilakukan dengan metode deskriptif dengan mengambil data lembar resep pasien diabetes melitus secara retrospektif. Dari 294 lembar resep yang memenuhi kriteria inklusi diperoleh 54,42% berjenis kelamin perempuan dan 45,58% berjenis kelamin laki-laki. Pasien dengan rentang usia 56–65 tahun lebih rentan terhadap penyakit diabetes melitus yang disertai dengan penyakit komorbid CAD. Terdapat 58,84% lembar resep yang mengalami potensi interaksi obat, dimana pada tingkat keparahan yang tertinggi terjadi pada tingkat keparahan moderate yaitu 473 kasus (87,11%). Obat yang paling banyak berpotensi menyebabkan interaksi obat adalah glimepirid dengan metformin sebanyak 88 kasus (16,21%); glibenklamid dengan aspirin sebanyak 28 kasus (5,18%); glimepirid dengan aspirin sebanyak 80 kasus (14,92%); metformin dengan furosemid sebanyak 31 kasus (5,71%). / Drug interaction is a problem related to drug therapy and can affect the clinical treatment outcomes of patients. Drug interactions can occur because of polypharmacy, patients who have acute or comorbid disease and patients receiving stronger drugs with narrow therapeutic indexes. This study aims to describe the potential interactions of oral antidiabetic drugs, to describe the quantity of oral antidiabetic prescriptions, and to determine the severity level of the potential interactions of oral antidiabetic drugs on prescription sheets for type 2 diabetes mellitus patients with comorbidities in the Outpatient Pharmacy Installation period January–June 2021. This study was conducted using a descriptive method by retrospectively taking data on prescription sheets for diabetes mellitus patients. Of the 294 prescription sheets that met the inclusion criteria, 54.42% were female and 45.58% were male. Patients with an age range of 56–65 years are more susceptible to diabetes mellitus accompanied by comorbid CAD. There were 58.84% of prescription sheets that experienced potential drug interactions, where the highest severity level occurred at moderate severity, namely 473 cases (87.11%). Drugs that have the most potential to cause drug interactions are glimepiride with metformin in 88 cases (16.21%); glibenclamide with aspirin in 28 cases (5.18%); glimepiride with aspirin in 80 cases (14.92%); metformin with furosemide in 31 cases (5.71%).
