Abstract

Latar Belakang: Otomikosis adalah infeksi pada kanalis auditorius eksterna (KAE) disebabkan oleh Jamur. Aspergillus sp dan Candida sp merupakan jamur terbanyak yang menyebabkan otomikosis. Kondisi cuaca Indonesia yang tropis ini menjadi salah satu faktor resiko terjadinya otomikosis karena suhu di Indonesia sekitar 20-35o membuat jamur mudah berkembang biak dengan baik. Pemberian tatalaksana otomikosis dengan pemberian obat anti jamur. Asam asetat 2% merupakan salah satu jenis obat anti jamur non spesifik untuk tatalaksana otomikosis yang mudah didapat. Saat ini, Rumah Sakit Umum Siloam Karawaci hanya menggunakan Asam asetat 2% dengan cara diirigasikan pada liang telinga untuk terapi otomikosis. Asam asetat 2% akan mengubah pH dalam liang telinga dan akan membuat membersihkan telinga dari mikroorganisme yang menyebabkan otomikosis . Tujuan Penelitian: Penelitian ini bertujuan untuk mengetahui prevalensi perbaikan gejala gatal, telinga penuh, keluar cairan, penurunan pendengaran, dan telinga berdengung pada pasien otomikosis di Rumah Sakit Umum Siloam Karawaci yang menggunakan pengobatan Asam Asetat 2%dengan cara irigasi. Metode Penelitian: Penelitian ini merupakan penelitian analitik kategorik tidak berpasangan dengan studi kohort, membutuhkan sampel sebanyak 39 peserta yang akan dilakukan dengan cara purposive pada pasien otomikosis yang telah memenuhi kriteria inklusi dan eksklusi dan pengambilan data yang dilakukan pada bulan Februari – April 2020 di Rumah Sakit Umum Siloam Karawaci. Hasil: Dari 39 responden yang memenuhi kriteria subyek penelitian, menurut perhitungan statistik didapatkan nilai p value 0,021 atau p < 0,05 pada gejala klinis gatal dengan responden yang masih mengalami gejala sebanyak 15,38%; p value 0,031 atau p < 0,05 pada gejala klinis telinga penuh dengan responden yang masih mengalami gejala 12,82%; p value 0,029 atau p <0,05 pada gejala klinis keluarnya cairan dengan responden yang masih mengalami gejala sebanyak 15,38%; p value 0,017 atau p < 0,05 pada gejala klinis penurunan pendengaran dengan responden yang masih mengalami gejala 23,08%; dan p value 0,023 atau p < 0,05 pada gejala klinis telinga penuh dengan responden yang masih mengalami gejala 12,82% setelah melakukan 14 hari pengobatan asam asetat2% secara irigasi. Kesimpulan: Dapat disimpulkan bahwa pemberian asam asetat 2% secara irigasi merupakan cara yang efektif untuk perbaikan gejala pada otomikosis ini karena pH asam asetat akan membunuh jamur yang ada didalam telinga dan cara dengan cara irigasi akan membersihkan liang telinga dari jamur. Asam asetat 2% juga mampu memperbaiki gejala yang dirasakah oleh responden. Kata Kunci: Otomikosis • Asam Asetat / Background: Otomycosis is an external auditory canal infection (KAE) caused by a fungus. Aspergillus sp and Candida sp are the most fungi that cause otomycosis. The condition of Indonesia's tropical weather is one of the risk factors for otomycosis because the temperature in Indonesia around 20-35o makes the fungus easily reproduce well. Provision of management of otomycosis by administering antifungal drugs. Acetic acid 2% is one type of non-specific antifungal drugs for the management of otomycosis that is easily obtained. Siloam Karawaci General Hospital only uses acetic acid 2% by irrigating the ear canal for the treatment of otomycosis. Acetic acid 2% will change the pH in the ear canal and will make cleaning the ear of microorganisms that cause otomycosis. Research Objectives: This study aims to determine the prevalence of improvement in symptoms of itching, ear fullness, discharge, decreased hearing, and ringing in the ears in otomycosis patients at Siloam Karawaci General Hospital who used 2% Acetic Acid treatment by irrigation. Research Methods: This study is a non-paired analytic categorical study with a cohort study, requiring a sample of 39 participants to be carried out by purposive sampling on otomycosis patient who have met the inclusion and exclusion Karawaci General Hospital. Results: From 39 respondents who met the criteria of research subjects, according to statistical calculations, a p value of 0.021 or p <0.05 was obtained for clinical symptoms of itching with 15.38% of respondents still experiencing symptoms; p value 0.031 or p <0.05 on clinical symptoms of full ear with respondents who still experience symptoms 12.82%; p value 0.029 or p <0.05 on clinical symptoms of discharge with 15.38% of respondents still experiencing symptoms; p value 0.017 or p <0.05 on clinical symptoms of hearing loss with respondents who are still experiencing symptoms 23.08%; and p value 0.023 or p <0.05 on clinical symptoms of full ear with respondents who still experience symptoms of 12.82% after 14 days of treatment with 2% acetic acid by irrigation. Conclusions : It can be concluded that the administration of 2% acetic acid by irrigation is an effective way to improve symptoms in this otomycosis because the pH of acetic acid will kill the fungus in the ear and irrigation will clean the ear canal from fungus. Acetic acid 2% is also able to improve the symptoms felt by the respondents. Keywords: Otomikosis • Acetic acid
