Abstract

Acne vulgaris, a disease of chronic inflammation on pilosebaceous unit, is one of the most common skin problems. Body mass index (BMI) could be an indicator in determining the risk of developing acne vulgaris – a high BMI is closely related to hyperinsulinemia, rise in androgen hormone dihydrotestosterone, and dyslipidemia, where each of these factors play a role in initiating and exaggerating the occurence of acne vulgaris, while a low BMI is associated with the other way around. The purpose of this research is to findout the association between body mass index and acne vulgaris on students of Faculty of Medicine Universitas Pelita Harapan, that is the occurence of acne which is further compared in terms of its degree of severity which is graded according to Global Acne Grading System. This research was an unpaired categorical comparative analytical study with crosssectional design which was conducted in Universitas Pelita Harapan from March to May 2019. Sampling was done through questionnaire using non-random purposive sampling method, with restriction of inclusion and exclusion criteria. Data was analyzed using Fisher’s exact testing. Data result from 212 respondents shows 208 respondents have mild acne, 4 respondents have moderate acne, 121 respondents have normal BMI, 91 respondents have high BMI. Bivariate analysis shows no significant association between body mass index and acne vulgaris (p=0,212; RR=3,989; 95% CI=0,422 – 37,725) / Akne vulgaris, suatu penyakit peradangan kronis pada unit pilosebasea, merupakan salah satu masalah kulit yang paling sering terjadi. Indeks massa tubuh (IMT) dapat menjadi indikator dalam menentukan risiko mengalami akne vulgaris, karena IMT yang tinggi berkaitan dengan tingginya kadar insulin, hormon androgen dihydrotestosterone, dan dislipidemia, di mana ketiga faktor ini saling berperan dalam menimbulkan terjadinya akne vulgaris dan memperparah derajat akne, sementara IMT yang rendah berkaitan dengan sebaliknya. Tujuan penelitian ini adalah untuk mengetahui hubungan antara indeks massa tubuh dengan akne vulgaris pada mahasiswa Fakultas Kedokteran Universitas Pelita Harapan, yaitu kejadian akne yang dibandingkan berdasarkan derajat keparahannya yang dinilai berdasarkan Global Acne Grading System. Penelitian ini merupakan studi analitik komparatif kategorik tidak berpasangan dengan desain studi potong lintang yang dilakukan di Universitas Pelita Harapan pada bulan Maret hingga Mei 2019. Metode pengambilan sampel dengan mengisi kuesioner dengan cara pencuplikan purposif dengan restriksi berupa kriteria inklusi dan eksklusi. Data dianalisis menggunakan uji statistik Fisher’s exact. Hasil penelitian dari 212 responden menunjukkan bahwa 208 responden memiliki akne derajat ringan, 4 responden memiliki akne derajat sedang, 121 responden memiliki IMT normal, 91 responden memiliki IMT tinggi. Hasil analisis bivariat menunjukkan bahwa indeks massa tubuh tidak memiliki hubungan yang signifikan dengan kejadian akne vulgaris (p=0,212; RR=3,989; 95% CI=0,422 – 37,725).
