Abstract
Latar Belakang: Kanker payudara merupakan kanker yang paling sering terjadi oleh wanita dengan jumlah terbanyak di dunia. Menurut Global Cancer Incidence, Mortality, and Prevalence, pada tahun 2020 angka kejadian kanker payudara mencapai 2.261.149 kasus kanker payudara baru pada wanita di seluruh dunia. Di Indonesia, terutama di Rumah Sakit MRCCC Siloam Semanggi belum pernah dilakukan penelitian mengenai hubungan antara faktor klinikopatologi dengan profil imunohistokimia pada kanker payudara. Tujuan Penelitian: Mengetahui hubungan antara faktor klinikopatologi dengan profil imunohistokimia (ER, PR, HER2, dan Ki-67) pada karsinoma payudara di Rumah Sakit MRCCC Siloam Semanggi pada tahun 2018-2021. Metode Penelitian: Penelitian akan dilakukan dengan desain analitik retrospektif, potong lintang. Sampel dipilih dengan teknik purposive sampling sebanyak 42 pasien kanker payudara di MRCCC Siloam Hospital Semanggi periode 2018 – 2021. Data diperoleh dari arsip Laboratorium Patologi Anatomi. Hasil: 88 sampel dikelompokkan untuk mengetahui status ekspresi hormonal dan hubungannya dengan faktor klinikopatologi. Hasil dalam penelitian ini mendapatkan 64,8%, 51,1%, 54,5%, dan 84,1% tingkat ekspresi reseptor estrogen (ER), reseptor progesteron (PR), HER-2, dan Ki-67. Terdapat hubungan yang signifikan antara ukuran tumor dengan ekspresi ER dan PR (p=0,039 dan p=0,003). Namun, tidak ada hubungan yang signifikan secara statistik antara ukuran tumor terhadap ekspresi HER-2 dan Ki-67 (p=0,303 dan p=0,061). Hubungan antara derajat histologi terhadap ekspresi ER, PR, dan Ki-67 juga didapati hasil yang signifikan dengan nilai berturut-turut (p=0,001, p=0,004 dan p=0,017). Namun, pada HER-2 tidak didapati hasil yang signifikan dengan nilai p=0,193. Tidak ada hubungan yang signifikan antara status usia dan metastasis kelenjar getah bening yang ditemukan dengan ekspresi reseptor. Kesimpulan: Dalam penelitian ini ditemukan adanya hubungan yang signifikan antara ukuran tumor dengan ER dan PR, begitu juga dengan derajat histologi yang terdapat hubungan dengan ER, PR, dan Ki-67. Kata Kunci: Faktor Klinikopatologi, HER2, Ki-67, Reseptor Estrogen, Reseptor Progesteron / Background: Breast cancer is the most common cancer in women with the highest number in the world. According to the Global Cancer Incidence, Mortality, and prevalence in 2020, the incidence of breast cancer reached 2,261,149 new breast cancer cases in women worldwide. In Indonesia, there are still few studies on the relationship between clinicopathological factors and immunohistochemistry profile in breast cancer, especially at MRCCC Siloam Semanggi Hospital. Aim: Knowing the relation between clinicopathological factors and immunohistochemistry profile (ER, PR, HER2, and Ki-67) in breast carcinoma at MRCCC Siloam Semanggi Hospital in 2018-2021. Methods: The study will be conducted with a retrospective analytical, cross-sectional research design. The sample was selected using a purposive sampling technique as many as 42 breast cancer patients at the MRCCC Siloam Hospital Semanggi outpatient installation for the period 2018-2021. Data were obtained from archive Anatomical Pathology Laboratory. Results: 88 samples were grouped to determine the status of hormonal expression and its relationship with clinicopathological factors. The results in this study obtained 64.8%, 51.1%, 54.5%, and 84.1% expression levels of estrogen receptor (ER), progesterone receptor (PR), HER-2, and Ki-67. There was a significant relationship between tumor size and ER and PR expression (p=0.039 and p=0.003). However, there was no statistically significant relationship between tumor size on HER-2 and Ki-67 expression (p=0.303 and p=0.061). There was also a significant relationship between the histological grade on expression of ER, PR, and Ki-67 (p=0.001, p=0.004 and p=0.017). However, in HER-2 there were no significant results with a value of p = 0.193. No significant association between lymph node metastatic status and age was found with receptor expression. Conclusions: In this study, it was found that there was a significant relationship between tumor size with ER and PR, as well as histological grade which had a relationship with ER, PR, and Ki-67.
