Abstract

Latar Belakang: Celah orofasial dirujuk sebagai celah bibir atau bibir sumbing (cleft lip) dengan atau tanpa celah langit (cleft palate) dan celah langit. Prevalensi dari celah orofasial di seluruh dunia terjadi pada 1 dari 700 bayi yang baru lahir. Celah orofasial juga menjadi salah satu kelainan pada kepala dan leher yang paling sering terjadi. Obstructive Sleep Apnea / OSA adalah salah satu jenis gangguan pernapasan pada tidur yang paling umum terjadi, ditandai dengan adanya pernapasan yang abnormal saat tidur. Pada anak-anak, abnormalitas kraniofasial menjadi salah satu faktor resiko dari OSA. OSA yang tidak terdiagnosa dan tidak ditangani dengan baik dapat menyebabkan berbagai macam masalah pada anak-anak. Tujuan: Tujuan dari penelitian ini adalah untuk mengetahui apakah terdapat hubungan antara celah bibir dan lelangit dengan obstructive sleep apnea pada pasien usia 2-18 tahun di Rumah Sakit Siloam, Karawaci. Metode Penelitian: Penelitian yang dilakukan adalah studi analitik komparatif kategorik tidak berpasangan dengan desain studi cohort. Sampel penelitian diambil dengan cara convenience sampling pada pasien dengan celah bibir dan lelangit usia 2-18 tahun dan pasien kontrol, dengan jumlah sampel sebanyak 132 sampel yang sesuai dengan kriteria inklusi dan eksklusi. Pengambilan data dilakukan menggunakan Pediatric Sleep Questionnaire. Hasil: Dari sebanyak 132 pasien dengan celah bibir dan lelangit serta pasien kontrol yang berpartisipasi dalam penelitian ini, didapatkan 38 (57,6 %) anak-anak dengan celah bibir dan lelangit dan 19 (28,8 %) anak-anak kontrol memiliki resiko OSA positif. Uji Chi-Square yang dilakukan menghasilkan hubungan yang signifikan antara celah bibir dan lelangit dengan OSA (p-value: 0,002 , p < 0,05). Kesimpulan: Terdapat hubungan yang signifikan antara pasien dengan celah bibir dan lelangit dengan OSA pada anak-anak usia 2-18 tahun. Anak-anak dengan celah bibir dan lelangit lebih beresiko mengalami OSA dibandingkan dengan anak-anak tanpa celah bibir dan lelangit. / Background: Orofacial clefts are referred to as cleft lip with or without cleft palate and cleft palate only. The global prevalence of orofacial clefts worldwide occurs in 1 in 700 births. Orofacial clefts are also one of the most common disorders of the head and neck. Obstructive Sleep Apnea / OSA is one of the most common type of breathing disorders in sleep, characterized by abnormal breathing during sleep. In children, craniofacial abnormalities are a risk factor for OSA. OSA that is not diagnosed and treated properly can cause various problems in children. Objective: The purpose of this study is to find out the relationship between cleft lip and cleft palate with obstructive sleep apnea in patients aged 2-18 years old at Siloam Hospital, Karawaci. Methods: The research is an unpaired categorical comparative analytic study with a cohort study design. The sample that is needed taken by convenience sampling in patients with cleft lip and cleft palate aged 2-18 years old and control group, with total of 132 samples that fit the inclusion and exclusion criteria. Data collection carried out using the Pediatric Sleep Questionnaire. Results: Of the 132 patients with cleft lip and palate as well as control patients who participated in this study, 38 (57.6%) children with cleft lip and palate and 19 (28.8%) children included in the control group have a positive OSA risk. The Chi-Square test that was performed produced a significant relationship between cleft lip and palate with OSA (p-value: 0.002, p <0.05). Conclusion: There is a significant relationship between patient with cleft lip and palate with OSA in children aged 2-18 years. Children with cleft lip and palate are more at risk of having OSA than children without cleft lip and palate.
