Abstract

Latar Belakang: ASI atau Air Susu Ibu adalah makanan alamiah berupa cairan yang diproduksi oleh manusia selama masa kehamilan. Lama menyusui adalah total hari yang dihitung dari awal hingga akhir masa menyusui. Salah satu akibat fatal dari kurangnya pemberian ASI ialah kondisi malnutrisi pada balita. Menurut Data Badan Kependudukan dan Keluarga Berencana Nasional (BKKBN) mencatat bahwa pada tahun 2021 terdapat 28.217 PUS (Pasangan Usia Subur) yang merupakan peserta KB aktif, dan lebih dari separuhnya yaitu sebesar 68,62% menggunakan metode kontrasepsi suntikkan. Terdapat dua penelitian yang menyatakan bahwa penggunaan kontrasepsi progestin dapat menurunkan produksi ASI bahkan diantaranya menyatakan berpeluang hingga 0.5 kali lipat menurunkan produksi ASI. Tujuan: Untuk mengetahui apakah terdapat hubungan antara penggunaan kontrasepsi progestin dengan lama menyusui pada puskesmas Kota Tangerang tahun 2023. Metode: Bahan yang digunakan dalam penelitian adalah lembar kuesioner yang berisikan informed consent, data pribadi, kuesioner, data anak, kriteria eksklusi dan inklusi, serta data penelitian. Wawancara kuesioner dilakukkan kepada Ibu Menyusui yang menggunakan kontrasepsi progestin dan memiliki anak usia 2-5 tahun. Hasil penelitian ini kemudian akan diolah dengan Microsoft Excel dan dianalisis menggunakan progtam Statistical Package for the Social Sciences (SPSS) 26.0. Hasil Penelitian: Berdasarkan hasil penelitian menunjukkan p=0.010 untuk kontrasepsi progestin jenis implan dan 0.016 jenis suntik 3 bulan sedangkan p=0.196 untuk kontrasepsi progestin jenis pil. Level signifikansi yang digunakan (p<0.05) dengan nilai OR (95% CI = 1.290 (0.504-3.304). Kesimpulan: Terdapat hubungan antara penggunaan kontrasepsi progestin implan dan suntik 3 bulan dengan lama menyusui pada puskesmas Kota Tangerang tahun 2023 dan tidak terdapat hubungan antara penggunaan kontrasepsi jenis pil dengan lama menyusui pada puskesmas Kota Tangerang 2023. Kata Kunci: Kontrasepsi, Hormonal, Kontrasepsi Progestin, Lama Menyusui, ASI Ekslusif. / Background: Breast milk or Mother's Milk is a natural food in the form of liquid produced by humans during pregnancy. Breastfeeding duration is the total days counted from the beginning to the end of the breastfeeding period.. One of the fatal consequences of the lack of breastfeeding is the condition of malnutrition in infants. According to data from the National Population and Family Planning Agency (BKKBN), in 2021 there were 28,217 PUS (Couples of Reproductive Age) who were active family planning participants, and more than half, namely 68.62%, used the injectable contraceptive method. There are two studies which state that the use of progestin contraceptives can reduce breast milk production, some of which are even stated to have a 0.5-fold chance of reducing breast milk production. Objective: To find out whether there is a relationship between the use of progesterone contraceptive at the Tangerang city health center in 2023. Methods: The materials used in the research were questionnaire sheets containing informed consent, personal data, questionnaires, child data, exclusion and inclusion criteria, and research data. Questionnaire interviews were conducted with breastfeeding mothers who used progestin contraception and had children aged 2-5 years. The results of this research will then be processed using Microsoft Excel and analyzed using the Statistical Package for the Social Sciences (SPSS) 26.0 program. Results: Based on the results, the study showed p=0.010 for implantable progestin contraception and 0.016 3-month injectable type whereas p=0.196 for pill-type progestin contraception. Significance level was used (p<0.05) with OR value (95% CI = 1.290 (0.504-3.304). Conclusion: There are significant results between the use of progestin implant contraception and 3-month injections and the duration of breastfeeding at Tangerang City health centers in 2023 and there are no significant results between the use of pill type contraception and the duration of breastfeeding at Tangerang City health centers in 2023.
