Abstract

Hipertensi adalah peningkatan tekanan darah sistolik ≥ 140 mmHg dan juga peningkatan tekanan darah diastolik ≥ 90 mmHg. Hipertensi telah terbukti dapat meningkatkan risiko komplikasi dan akan menyebar ke berbagai macam organ target di dalam tubuh. Pasien hipertensi membutuhkan >1 obat pada pelaksanaan terapinya, yang akan menyebabkan terjadinya Drug Related Problems (DRPs). DRPs adalah suatu kondisi yang berkaitan dengan terapi obat dengan tujuan terapi secara optimal. Dinas Kesehatan provinsi Banten pada tahun 2019 menyatakan bahwa Tangerang menjadi satu di antara kabupaten/kota yang memiliki kasus hipertensi dengan jumlah tertinggi yaitu 622.060 kasus. Penelitian ini bertujuan mengetahui profil DRPs pada pasien hipertensi dengan stroke rawat jalan di Rumah Sakit X Tangerang periode 1 Oktober 2023 – 31 Desember 2023. Penelitian dilakukan dengan menggunakan data rekam medis pasien yang dianalisis lebih lanjut dengan menggunakan uji persentase dan didapatkan hasil bahwa pasien rawat jalan hipertensi dengan stroke di Rumah Sakit X Tangerang memiliki Drug Related ProblemS (DRPs) yaitu obat yang tidak tepat menurut pedoman/formula (C1.1) sebanyak 37 kasus (31%), tidak ada indikasi obat (C1.2) sebanyak 27 kasus (23%), perawatan obat tidak ada atau tidak lengkap meskipun ada indikasi (C1.5) sebanyak 25 kasus (22%), dosis obat terlalu rendah (C3.1) sebanyak 18 kasus (18%), regimen dosis terlalu sering (C3.4) sebanyak 8 kasus (8%), dan instruksi dosis waktu salah, tidak jelas atau hilang (C3.5) sebanyak 1 kasus (1%)./Hypertension is an increase in systolic blood pressure ≥ 140 mmHg and also an increase in diastolic blood pressure ≥ 90 mmHg. Hypertension has been proven to increase the risk of complications and will spread to various target organs in the body. Thus, hypertensive patients need more than 1 drug in their therapy, which will increase the potential for Drug Related Problems (DRPs). DRPs are a condition related to drug therapy with the aim of optimal therapy. The Banten provincial Health Service in 2019 stated that Tangerang was one of the districts/cities that had the highest number of hypertension cases, namely 622,060 cases. This study aims to determine the profile of DRPs in hypertensive patients with outpatient stroke at Hospital X Tangerang for the period 1 October 2023 - 31 December 2023. The research was carried out using patient medical record data which was further analyzed using a percentage test and the results showed that hypertensive outpatients with stroke at Hospital X Tangerang had Drug Related Problems (DRPs), namely Inappropriate medication according to guidelines/formula (C1.1) was 37 cases (31%), there was no indication for medication (C1.2) in 27 cases (23%), drug treatment was absent or incomplete despite indications (C1.5) in 25 cases (22%), the drug dose was too low (C3.1) in 18 cases (18%), too frequent dosing regimen (C3.4) in 8 cases (8%), and the dosing instructions at the time were incorrect, unclear or missing (C3.5) in 1 case (1%).
