Abstract

Diabetic ulcers are a complication of Diabetes Mellitus with prevalence of 15-25% during their lifetime and diabetic ulcers are a major cause of amputations in the lower extremities due to slow wound healing and susceptibility to infections caused by neurological and vascular disorders. Depression can occur as a result of lifestyle changes, economic burden, length of treatment, uncontrolled high sugar, a sense of dependence on others, and also the progressiveness of ulcers. Research that raised the topic of the relationship of diabetic ulcer degree to depression level has been done, but the results obtained quite diverse and contradictory, it is important to do further research. The purpose of this study was to find out whether there was a relationship between the degree of diabetic ulcers against the severity of depression levels. This research use cross-sectional design, with primary data taken by interview. The sample consists of 35 patients general surgical polyclinic and ward of Rumah Sakit Umum Siloam Lippo Karawaci. Data collection was done on March 2018 to May 2018. Ethics was approved by Mochtar Riady Institute of Nanotechnology and related parties in the intended hospital. Data processing was done with SPSS 22 software and statistical review was done using Chi Square. Based on the statistical review, there is correlation between degree of diabetic ulcers and stage of depression (p = 0.03). This result may be due to several factors such as decreasing self esteem, unpleasant smell, disappointment and failure, decreasing appetite, weight loss, duration of sleep, tiredness, and relation of the patient with their surroundings. The conclusion of this study is there is correlation between degree of diabetic ulcers and stage of depression / Ulkus diabetikum merupakan sebuah komplikasi dari Diabetes Melitus dengan prevalensi 15- 25% semasa hidupnya dan merupakan penyebab utama amputasi pada ekstremitas bawah karena penyembuhan luka yang lambat dan kerentanan terhadap infeksi yang disebabkan oleh gangguan neurologis dan vaskuler. Depresi dapat terjadi akibat dari perubahan pola hidup, beban ekonomi, lamanya pengobatan, gula darah yang tidak terkontrol, rasa ketergantungan terhadap orang lain, dan juga memberatnya ulkus. Penelitian yang mengangkat topik mengenai hubungan derajat ulkus diabetikum terhadap tingkat depresi sudah pernah dilakukan, namun hasil yang didapatkan cukup beragam dan bertentangan, maka penting untuk dilakukan penelitian lebih lanjut. Tujuan dari penelitian ini adalah untuk megetahui apakah terdapat hubungan antara derajat ulkus diabetikum terhadap tingkat depresi. Penelitian ini menggunakan metode potong lintang dengan pengambilan data primer dengan metode wawancara. Sampel berjumlah 35 orang pasien poli bedah dan bangsal Rumah Sakit Umum Siloam. Pengambilan data berlangsung dari Maret 2018 hingga Mei 2018. Etika persetujuan penelitian telah disetujui oleh Mochtar Riady Institute of Nanotechnology dan pihak terkait di rumah sakit tertuju. Pengolahan data penelitian diolah menggunakan software SPSS 22 dan uji statistik menggunakan metode Chi Square. Hasil uji statistik ditemukan adanya hubungan antara derajat ulkus diabetikum terhadap tingkat depresi (p = 0.003). Hal ini dipengaruhi oleh dipengaruhi oleh beberapa faktor yakni adanya penurunan kepercayaan diri, bau tidak sedap, rasa kecewa dan gagal terhadap diri sendiri, penurunan nafsu makan, berat badan, waktu tidur, rasa mudah lelah, dan juga hubungan pasien dengan orang-orang disekitarnya. Kesimpulan dari penelitian ini adalah terdapat hubungan antara derajat ulkus diabetikum dan tingkat depresi.
