Abstract

Background Acute appendicitis is the most common abdominal emergency requiring surgical treatment. Occurance in misdiagnosing of acute appendicitis still happens leading in increased negative appendectomy rate of 15-40%. Therefore, early and accurate diagnosis is needed in order to decrease these morbidity, mortality, and negative appendectomy rates. The Alvarado score is a simple scoring system utilized to determine the likelihood of acute appendicitis based on history examination, physical examination, and laboratory tests. Aim To know the sensitivity of Alvarado score in histopathologic findings of acute appendicitis in Rumah Sakit Umum Siloam. Material and Methods This analytical cross-sectional study design was conducted in Rumah Sakit Umum Siloam from April until June 2018. The complete clinical history and histopathologic findings of patients with suspected acute appendicitis were collected from medical record. Statistical test was done by using diagnostic test to find sensitivity, chi-square for data distribution, and ROC curve to evaluate the performance of Alvarado score sensitivity by using SPSS v23. Results Out of 60 patients. 46.7% had Alvarado score >7 and 53.3% had Alvarado score <7. The sensitivity of Alvarado score are 52.94%. The AUC of Alvarado score to diagnose acute appendicitis was found 0.671 (AUC>0.7) Conclusion Alvarado score is not sensitive based on histopathological findings in Rumah Sakit Umum Siloam / Latar Bclakang Apcndisitis akut mcrupakan akut abdomen yang mcmerluknn lindaknn pembedahan darurat paling umuin. Diagnosis apcndisitis akut mnsih terdapat kesalahan dengan angka apendektomi negatif sekitar 15 - 40%. Karena itu, diagnosis awnl dan akurat dibutuhkan untuk mengurangi angka morbiditas. mortalitas, dan apendektomi negatif. Skor Alvarado mcrupakan sebuah sistem skoring digunakan untuk menentukan kemungkinan apcndisitis akut berdasarkan anamnesis, pcmcriksaan fisik, dan pemeriksaan laboralorium. Tujuan Mengetahui sensitivitas skor Alvarado pada temuan histopatologi apcndisitis akut di Rumali Sakit Umiun Siloam. Bahan dan Mctodc Studi potong lintang analitik ini dilakukan di Rumah Sakit Umuin Siloam pada April sampai Juni 2018. Uasil pemeriksaan klinis dan temuan histopatologis pasien apcndisitis akut dikumpulkan dari rekam medis. Uji statistik menggunakan uji diagnostik untuk mcncari sensitivitas. uji chi square untuk melihat signifikansi distribusi data, dan kurva ROC untuk mencari AUC untuk menilai sensitifnya skor Alvarado dengan menggunakan SPSS v23. Hasil Dari 60 pasien. ditemukan 46.7% pasien dengan skor Alvarado >7 dan 53.3% dengan skor Alvarado <7. Skor Alvarado memiliki sensitivitas 52.94%. AUC skor Alvarado untuk mendiagnosa apcndisitis akut didapatkan sebesar 0.671 (AUC>0.7). Kcsimpulan Skor Alvarado tidak sensitif untuk mendiagnosa apcndisitis akut di Rumah Sakit Umum Siloam.
