Abstract

Latar Belakang: Dismenore merupakan salah satu gangguan ginekologis yang paling umum. Prevalensi umum dismenore primer adalah sekitar 75%. Indeks massa tubuh underweight (IMT <18,5) dan berlebih (IMT ≥23) menjadi salah satu faktor risiko yang dapat mempengaruhi derajat dismenore primer. Beberapa penelitian juga menunjukkan terdapat hubungan antara usia menarke dan aktivitas fisik dengan derajat dismenore primer. Tujuan: Mengetahui hubungan antara status gizi berdasarkan IMT dan faktor-faktor determinan lain dengan derajat dismenore primer. Metodologi: Desain studi potong lintang dengan sampel 198 subjek dan menggunakan teknik purposive sampling. Pengambilan data menggunakan wawancaran kuesioner dan pengukuran tinggi serta berat badan. Hasil penelitian diolah dengan analisis Chi – Square dan regresi logistik. Hasil: Hubungan IMT underweight terhadap derajat dismenore primer setelah mengendalikan faktor usia menarke dan aktivitas fisik tampak bermakna secara klinis maupun statistik (OR adjusted = 3,23, p = 0,02, 95% CI = 1,16 – 8,95). Tidak terdapat perbedaan probabilitas antara kelompok overweight at risk dengan normal secara klinis dan statistik. Secara klinis, terlihat peningkatan probabilitas pada kelompok obesitas tipe 1 dan tipe 2 dibandingkan IMT normal, yaitu (p = 0,60, OR = 1,55, 95% CI = 0,51 – 4,75) dan (p = 0,47, OR = 2,12, 95% CI = 0,41 – 10,9). Usia menarke dan aktivitas fisik berhubungan secara klinis dengan derajat dismenore primer, namun secara statistik tidak signifikan (p = 0,24, OR = 1,63, 95% CI = 0,79 – 3,34) dan (p = 0,10, OR = 1,77, 95% CI = 0,94 – 3,35). Kesimpulan: Terdapat hubungan yang signifikan pada IMT underweight dan obesitas tipe 2 terhadap derajat dismenore primer. Usia menarke dan aktivitas fisik berkontribusi terhadap derajat dismenore primer. Kata Kunci: Indeks Massa Tubuh, Faktor Determinan Lain, Derajat Dismenore Primer/ Background: Dysmenorrhea is one of the most common gynecologist problem. The general prevalention of primary dysmenorrhea is estimated 75%. Underweight body mass index (BMI <18,5) and overweight ( BMI ≥23) is become one of the risk factor that would affected the degree of primary dysmenorrhea. Some research also showed the relation between menarche age and physical activity with the degree of primary dysmenorrhea. Aim:To understand the relation between nutrition status based on BMI and the other determined factors with degree of primary dysmenorrhea. Methodology: The relation of underweight BMI to primary dysmenorrhea degree after controlling the menarche age factor and physical activity is looked meaningful neither clinically or statistically (OR adjusted = 3,23, p = 0,02, 95% CI = 1,16 – 8,95). There is no probability different between overweight at risk group with normal group neither clinically or statistically. Clinically, the increasing probality is showed in type 1 and type 2 obesity group compared with normal BMI, which is (p = 0,60, OR = 1,55, 95% CI = 0,51 – 4,75) and (p = 0,47, OR = 2,12, 95% CI = 0,41 – 10,9). The menarche age and physical activity is clinically related with primary dysmenorrhea degree, however statistically not significant (p = 0,24, OR = 1,63, 95% CI = 0,79 – 3,34) and (p = 0,10, OR = 1,77, 95% CI = 0,94 – 3,35). Conclusion: There is a significant relation on underweight BMI and obese type 2 to the primary dismenore degree. The menarche age and physical activity is contributed to the primary dysmenorrhea degree. Key Words: Body Mass Index, Other Determined Factor, Degree of Primary Dysmenorrhea
