Abstract

Latar Belakang: Stroke merupakan salah satu penyakit yang terus mengalami peningkatan angka kejadian di Indonesia. Stroke iskemik merupakan salah satu tipe stroke yang paling sering ditemui. Stroke iskemik dapat menyebabkan gangguan fungsi kognitif. Fungsi kognitif melibatkan kemampuan seseorang untuk berpikir dan memproses informasi. Prevalensi pasien dengan riwayat stroke yang mengalami penurunan fungsi kognitif di Korea Selatan pada tahun 2012 adalah 69,8% sedangkan di Indonesia sendiri pada penelitian yang dilakukan di Manado didapatkan prevalensi pasien stroke yang mengalami gangguan fungsi kognitif sebesar 67,5%. Tujuan Penelitian: Untuk mengetahui hubungan antara lesi pada lobus frontal dengan fungsi kognitif pada pasien pasca stroke iskemik di Rumah Sakit Siloam Karawaci. Metode Penelitian: Penelitian ini merupakan penelitian analitik dengan menggunakan studi potong lintang. Besar sampel dalam penelitian sebanyak 50 pasien stroke iskemik yang mempunyai lesi stroke yang dibagi menjadi 25 pasien dengan lesi pada lobus frontal dan 25 pasien dengan lesi pada lobus lainnya. Pengambilan data dilakukan mulai dari Februari 2020 sampai dengan Maret 2020 dengan menggunakan CT Scan pasien untuk mengetahui lokasi lesi stroke serta kuesioner MoCA-Ina untuk penilaian fungsi kognitif. Uji statistik dilakukan dengan menggunakan aplikasi Statistical Package for the Social Sciences (SPSS) versi 25.0. Hasil: Terdapat hubungan yang signifikan antara lokasi lesi pada lobus frontal dengan fungsi kognitif pada pasien pasca stroke iskemik dengan p-value 0,024. Terdapat 3 komponen fungsi kognitif yang menunjukkan hasil yang signifikan yaitu visuospasial (p-value 0,044), atensi (p-value 0,08) dan bahasa (p-value 0,020). Kesimpulan: Terdapat hubungan antara lokasi lesi pada lobus frontal dengan penurunan fungsi kognitif pada pasien pasca stroke iskemik di Rumah Sakit Siloam Karawaci / Background: Stroke is one of the diseases which continues to advance in number of occurrence in Indonesia. Ischemic stroke is one of the type of strokes which is most often encountered. Ischemic stroke can cause cognitive dysfunction. The cognitive function involves a person’s ability to think and processing an information. The prevalence of patients with a history of stroke who experienced a decline in cognitive function in South Korea in 2012 was 69.8%, while in Indonesia a study conducted in Manado found the prevalence of stroke patients who experienced cognitive dysfunction was 67.5%. Objective: To determine the relation between frontal lesion with cognitive dysfunction in post-ischemic stroke patients at Siloam Karawaci Hospital. Method: This is an analytic study research which use cross sectional study. The sample size for this research is 50 ischemic stroke patients who had stroke lesion which will be divided into 25 patients with lesions in the frontal lobes and 25 patients with lesions in other lobes. Data collection was carried out from February 2020 until March 2020 using the patients’ CT Scan to find out the location of the stroke lesions as well as the MoCA-Ina questionnaire for the assessment of the cognitive function. Statistical tests were performed using the Statistical Package for The Social Sciences (SPSS) version 25.0. Results: There is a significant relationship between the locations of the lesion in the frontal lobe with the cognitive function in post-ischemic stroke patients with p-value of 0.024. There are three components of cognitive function that show significant results, i.e. visuospatial (p-value 0.024), attention (p-value 0.08), and language (p-value 0.020). Conclusion: There is a relation between the locations of the lesion in the frontal lobe with the decrease in cognitive function in the post-ischemic stroke patients at Siloam Karawaci Hospital.
