Abstract
Penyakit ginjal kronis (PGK) adalah masalah kesehatan global yang mengalami peningkatan prevalensi. Berdasarkan data Riskesdas, prevalensi penyakit ginjal kronis di Indonesia mengalami peningkatan dari 2% pada tahun 2013 menjadi 3.8% pada tahun 2018. Banyak penelitian sebelumnya menunjukkan berbagai faktor risiko penyakit ginjal kronis dapat menimbulkan penurunan fungsi kognitif namun belum ada yang menunjukkan faktor mana yang berpotensi paling buruk terhadap penurunan fungsi kognitif. Penelitian ini bertujuan untuk mengetahui hubungan dan besaran dampak yang ditimbulkan oleh faktor risiko PGK terhadap penurunan fungsi kognitif pasien dengan penyakit ginjal kronis.
Penelitian menggunakan desain cross sectional kepada 62 pasien hemodialisa RSUS Karawaci. Pengambilan data dilakukan menggunakan kuesioner MoCA versi Indonesia dan rekam medis pasien, lalu diolah dengan analisis chi-square dan multivariat. Dari hasil penelitian, didapatkan hubungan yang bermakna antara variable riwayat hipertensi, riwayat merokok, tingkat Pendidikan, dan usia lanjut terhadap penurunan fungsi kognitif dengan p-value masing-masing 0.004, <0.001, 0.017, dan 0.041. Analisis multivariat menunjukkan riwayat hipertensi memiliki pengaruh terbesar dengan p-value 0.001 (95% CI 4.087 – 347.177). Kesimpulan penelitian ini yaitu terdapat hubungan yang bermakna antara riwayat hipertensi, riwayat merokok, tingkat Pendidikan, dan usia lanjut terhadap penurunan fungsi kognitif dengan riwayat hipertensi memiliki pengaruh terbesar terhadap penurunan fungsi kognitif. / Chronic kidney disease (CKD) is global health problem with increasing prevalence. Based on Riskesdas, Indonesian CKD prevalence has increased from 2% in 2013 to 3.8% in 2018. Previous studies have shown various risk factors for CKD can cause a decline in cognitive function, but none about which factors have the most influence for cognitive decline. This study aims to determine the relationship and the magnitude caused by risk factors for CKD on cognitive function decline in CKD’s patients. The study used a cross sectional design for 62 hemodialysis patients at RSUS Karawaci. Data was collected using the Indonesian version of the MoCA questionnaire and medical record, then processed with chi-square and multivariate analysis. Results of this study show a significant relationship between variables such as hypertension, smoking, education level, and old age cognitive declining with p-values respectively 0.004, <0.001, 0.017, and 0.041. Multivariate analysis showed that history of hypertension had the greatest influence with p-value of 0.001 (95% CI 4,087 - 347,177). The conclusion of this study is that there is significant relationship between the hypertension, smoking, education level, and old age on the decline in cognitive function with hypertension having the greatest influence on the decline in cognitive function.
