PT. Matahari Putra Prima, Tbk., is one of Indonesia’s big company. As it’s vision suggest to be the number one retailer company in Indonesia, PT. Matahari Putra Prima, Tbk. tooks many ways in order to improve it’s quality of work. This Employee’s Medical Claim Application Design is an application that is used to record medical claim information of PT. Matahari Putra Prima, Tbk.’s employee and their family. This application program aims to give reports that contain information about how often an employee and it’s family make medical claims and how much they spend in a period of time. There are four steps needed in the making of this application program. The first step is to design a Login Form. This form is used to identify authorized users. The next step is making of Splash Form that shows information about the program, the name of the creator, and the company name. The third step is making of MDI Form that has three menu, they are Form Daftar_Karyawan, Form Daftar_Tertanggung, Form Daftar_Pengobatan. The last is to make Period Form, that is contains the report. This application program was made using Microsoft Visual Basic 6.0, Microsoft Access 2000, and Crystal Report 8.5. From here, we can get information about how often an employee and it’s family make medical claims and how much they spend in a period of time. However, this program still contains one drawback, that is the nonexistance of a clear total cost limit for one’s medical claim in a month of period. Moreover, by using Microsoft Visual Basic 6.0 as user interface between user and the program and Microsoft Access VisData as its database into full use, a program will be a user friendly one, with an interesting report design in Crystal Report.
