Abstract

Discharge planning is a systematic process in health care to assist patients and families in setting needs, implementing and coordinating a treatment plan that will be performed after the patient returns from the hospital. to know the description of the discharge planning process at Siloam Hospital General Pavilion. This study is a sample documentation study in this study amounting to 315 documents The results of this study look at the number of Discharge Planning process in the inpatient room of Siloam Hospital General Pavilion using Checklist sheet, to evaluate the process undertaken by the nurse. Total percentage was performed complete (6,6%), done incomplete (44,12%), and not done (48,68). With the majority of 179 as many as 56.82% on the question of the patient starting to be treated the number and the lowest 37 as much as 11.74% on the question of home care plans. Lack of Discharge Planning process is done in the home care plan with the amount of 11,74%. Based on research result and experience of this research, hence researcher wish to give suggestion for service of nursing and subsequent research to fill and complete Discharge Planning form sheet especially at home care plan, suggestion for next researcher need dilakuka research about nurse knowledge / Discharge planning adalah suatu proses yang sistematis dalam pelayanan kesehatan untuk membantu pasien dan keluarga dalam menetapkan kebutuhan, mengimplementasikan serta mengkoordinasikan rencana perawatan yang akan dilakukan setelah pasien pulang dari rumah sakit. Mengetahui gambaran proses discharge planning di Paviliun Umum Rumah Sakit Siloam. Penelitian ini merupakan studi dokumentasi sampel pada penelitian ini berjumlah 315 dokumen Hasil penelitian ini melihat jumlah proses Discharge Planning di ruang rawat inap Paviliun Umum Rumah Sakit Siloam dengan menggunakan lembar Checklist, untuk mengevaluasi proses yang dilakukan oleh perawat. Didapatkan jumlah persentase dilakukan lengkap (6,6%), dilakukan kurang lengkap (44,12%), dan yang tidak dilakukan (48,68). Dengan mayoritas terbanyak 179 sebanyak 56,82% pada pertanyaan tanggal pasien mulai dirawat jumlah dan terendah 37 sebanyak 11,74% pada pertanyaan rencana perawatan dirumah. Kurang dilaksanakan proses Discharge Planning dibagian rencana perawatan dirumah dengan jumlah 11,74%. Berdasarkan hasil penelitian dan pengalaman dari penelitian ini, maka peneliti ingin memberikan saran bagi pelayanan keperawatan dan penelitian selanjutnya untuk mengisi dan melengkapi lembar form Discharge Planning terutama di rencana perawatan dirumah, saran bagi peneliti selanjutnya perlu dilakuka penelitian tentang pengetahuan perawat.
