Abstract

Latar Belakang: Di Indonesia, diperkirakan 17,3 juta orang meninggal karena aterosklerosis, dengan 7,3 juta akibat penyakit jantung koroner dan 6,2 juta akibat stroke. Menurut Depkes Indonesia tahun 2010, angka aterosklerosis di Indonesia hampir sebanyak 50%. Pentingnya deteksi dini penyakit jantung koroner. Low dose CT toraks non kontras dapat menjadi metoda alternatif yang lebih praktis untuk mendeteksi penyakit jantung koroner melalui deteksi kalsifikasi koroner yang menggambarkan adanya proses aterosklerosis di arteri koroner. Saat ini belum ada penelitian yang menggambarkan prevalensi kalsifikasi koroner dan hubungannya dengan stenosis arteri koroner pada populasi Indonesia. Tujuan Penelitian: Penelitian ini dilakukan dengan tujuan untuk mengetahui apakah temuan kalsifikasi koroner pada pemeriksaan low dose CT toraks non kontras berhubungan dengan stenosis arteri koroner pada pemeriksaan angiografi koroner. Metode Penelitian: Penelitian ini merupakan studi observasional analitik dengan metode potong lintang. Subjek penelitian adalah pasien yang melakukan pemeriksaan low dose CT toraks non kontras dan yang juga melakukan pemeriksaan angiografi koroner invasif periode Januari 2020 sampai Desember 2020. Data penelitian diambil dari rekam medis pasien Rumah Sakit Siloam Lippo Village dan dilakukan analisis temuan kalsifikasi arteri koroner tiap segmen dengan pemeriksaan low dose CT toraks non kontras dan temuan stenosis arteri koroner tiap segmen dengan pemeriksaan angiografi koroner. Analisis data dilakukan dengan uji statistik Chi Square. Hasil: Penelitian ini menunjukkan bahwa kalsifikasi koroner ditemukan lebih banyak pada laki-laki, di pembuluh darah Left Anterior Descending dan stenosis arteri koroner juga ditemukan lebih banyak pada Left Anterior Descending. Terdapat hubungan yang signifikan antara kalsifikasi dengan stenosis pada pembuluh Left Main, Left Circumflex, dan Right Coronary Artery. / Background: In Indonesia, an estimated 17.3 million people die from atherosclerosis, with 7.3 million from coronary heart disease and 6.2 million from stroke. According to the Indonesian Ministry of Health in 2010, the rate of atherosclerosis in Indonesia was almost 50%. It is important for early detection of coronary heart disease. Low Dose CT non-contrast can be a more practical alternative method for detecting coronary heart disease through the detection of coronary calcification which reflects the presence of atherosclerosis develops in the coronary arteries. Until now, there is no research that describes the prevalence of coronary calcification and the relation with coronary artery stenosis for the Indonesian population. Objective: This study was conducted with the aim of finding out whether the findings of coronary calcification on low dose CT non-contrast examination were associated with coronary artery stenosis on coronary angiography examination. Method: This study is an analytical observational study with a cross-sectional method. The research subjects were patients who underwent low dose CT non-contrast examinations and also underwent invasive coronary angiography examinations for the period of January 2020 to December 2020. Research data was taken from medical records of patients at Siloam Lippo Village Hospital and analysis of coronary artery calcification findings for each segment was carried out using low dose CT non-contrast examination and findings of coronary artery stenosis in each segment using coronary angiography. Data analysis was carried out using the Chi Square statistical test. Results: This study shows that coronary calcification is found more in men, in the Left Anterior Descending blood vessels and coronary artery stenosis is also found more in the Left Anterior Descending. There is a significant relationship between calcification and stenosis in the Left Main, Left Circumflex and Right Coronary Arteries.
