Abstract

Tindakan bedah memiliki resiko tinggi seperti pendarahan, kerusakan jaringan tubuh, dan komplikasi pasca operasi. Untuk meminimalkan resiko tinggi serta meningkatkan mutu ketrampilan dokter bedah diperlukan pembelajaran langsung dan tidak langsung. Pembelajaran lewat video rekam bedah adalah bentuk pembelajaran tidak langsung berdasarkan kasus dan kondisi nyata. Kelebihan dari teknik ini adalah pembelajarannya lebih mudah, lebih bervariasi untuk berbagai kasus dan dapat diulang sesuai kebutuhan. Media rekaman dapat menjadi salah satu cara untuk berbagi pengalaman dokter bedah kepada dokter bedah lainnya. Rekaman tersebut juga dapat dijadikan bahan evaluasi untuk meningkatkan kualitas pelayanan kesehatan. Saat ini smartphone mempunyai kualitas kamera yang relatif baik dan potensial digunakan sebagai alat dokumentasi bedah di ruang operasi. Untuk menghasilkan rekaman yang ideal, dimana obyek dan proses bedah terekam secara utuh dan lengkap, diperlukan penggabungan tiga sudut padang rekam. Sudut padang pertama dan kedua didapatkan dari smartphone yang ditempatkan pada dahi dokter bedah dan asistennya dengan tujuan merekam proses obyek bedah dari sudut pandang dokter. Sudut pandang ketiga ditempatkan di atap ruangan untuk melihat keseluruhan aktivitas bedah. Konfigurasi perekaman dari ketiga smartphone dapat dikontrol menggunakan IPwebcam melalui PC (personal computer) atau laptop. IPWebcam adalah aplikasi yang dapat menghubungkan antara smartphone dan PC atau laptop melalui protokol WiFi untuk keperluan video streaming. Hasil dari ketiga smartphone tersebut digabungkan untuk memperoleh seluruh aktivitas bedah secara lebih komprehensif. Dari hasil rekaman berdasarkan 8 kasus aktivitas bedah yang terdapat pada media online diperoleh bahwa 87,5% kasus dapat terekam menggunakan metode ini. Berdasarkan tiga kali percobaan perekaman, IPwebcam dapat mengontrol tiga buah smartphone menggunakan protokol WiFi. Perekaman dilakukan menggunakan dua jenis video renderer, yaitu video renderer Javascript dan video renderer web browser. Sebagai kesimpulan, metode ini dapat merekam proses bedah secara lengkap dan utuh. Video renderer yang disarankan adalah video renderer web browser yang dapat memberikan warna gambar secara lebih stabil berkat kemampuan web browser mengkomposisi video secara lebih baik dan dalam pengisian piksel gambar. / Surgical process potentially has a high risk, such as bleeding, tissue damage, and post operation complication. To minimize surgical process’s risk, and to improve the surgeon’s skills and quality, direct and indirect learning are required. Surgical learning through video recording is an indirect form of learning, based on real cases and conditions. The advantages of this technique are easier to learn, more varied for various cases, and can be played back if needed. The video can be used as a medium for sharing experiences among physicians to improve their skill and service quality in doing surgical operation. The recording can also be used as an evaluation material to improve the health services quality. Nowadays, smartphone has a good camera quality and can be used as a documentation tool in the operating room. To produce an ideal recording, where the objects and surgical processes are recorded in a complete manner, a combination of three points of view is needed. The first and second point of view are obtained from the smartphone placed on the surgeon's and surgical assistant forehead. The purpose of this placement is to record the process of a surgical object from a doctor's point of view. The third point of view is placed on the roof of the operating room to see the overall surgical activity. The recording configuration of the three smartphones can be controlled using IPwebcam via PC or laptop computer. IPWebcam is an application used to connect between smartphone and PC via WiFi protocol for video streaming purpose. The results obtained from using those smartphones are combined to provide a more comprehensive understanding to the whole surgical activities. From the recordings which make use of Javascript video renderer and web browser, based on 8 cases of surgical activity contained in online media, it yields that 87.5% of the results can be recorded using this method. Besides, IPwebcam can control all smartphones using the WiFi protocol. As the conclusion, this method can be applied to record the whole surgical process completely. The preferable video renderer for use is the web browser renderer which may produce a more stable image color. This advantage is due to the fact that web browser has a better capability for composing video, as well as to fill image pixels.
