Abstract
Latar Belakang : Fibrosis Miokardium (FM) suatu penyakit yang diakibatkan karena proses remodeling cardiac dari Ekstraseluler matriks (ECM) dengan gambaran MRI-LGE.
Secara etiologi, fibrosis miokardium sebelum pandemi COVID-19, pada masa ini lebih sering terjadi karena adanya myocarditis akibat viral oleh adenovirus, hepatitis, herpes
simplex virus (HPV).1,4 Sedangkan, pada masa pandemi COVID-19 viral yang mengakibatkan myocarditis atau myocardial fibrosis adalah HPV, HIV, Influenza A dan B virus, dan SARS-CoV 2 virus. Menurut data epidemiologi global sebesar 17-24%. American Heart Association (AHA) menyatakan sebesar 59,6% dengan peningkatan 12,1% dari tahun-tahun sebelumnya terkait kejadian myocarditis. Sedangkan, Riset Kesehatan Dasar (Riskesdas) menunjukan prevalensi penyakit jantung yang meningkat 1,5% setiap tahunnya dan prevalensi miokarditis di Indonesia sebesar 29,2% selama 10 tahun terakhir.
Tujuan Penelitian: Mengetahui perbandingan temuan fibrosis miokardium pada MRI Jantung LGE sebelum dan sesudah pandemi COVID-19.
Hipotesis Penelitian: Terdapat perbandingan Fibrosis miokardium sebelum dan sesudah pandemi COVID-19.
Metode Penelitian: Penelitian ini merupakan studi analitik komparatif kategori berpasangan dengan metode potong lintang yang dikumpulkan secara retrospektif dengan data sekunder yaitu rekam medis bagi subjek yang melakukan pemeriksaan MRI Jantung LGE yang dipilih secara non-random menggunakan teknik purposive sampling dan dilakukan analisis menggunakan Chi-Square.
Hasil: Penelitian memperoleh 419 subjek yang memiliki prevalensi fibrosis miokardium sebelum pandemi COVID-19 sebesar 80,4% dan 87,6% sesudah pandemi COVID-19. Ditemukan perbandingan yang signifikan pada data tipe LGE dengan ischemic pattern tipe subendocardial (p: 0,016, OR: 1,7 ,95% CI: 1,102-2,622) dan tipe LGE dengan non-ischemic pattern tipe epicardial (p: <0,001, OR: 0,374 ,95% CI: 0,242-0,577). Prevalensi terbesar dimiliki oleh tipe midwall dan patchy myocardial sesudah pandemi COVID-19 sebesar 39,7% (p: 0,093, OR: 0,802-17,442, 95% CI: 3,74) dibandingkan sesudah pandemi COVID-19 tipe transmural sebesar 29,9% (p: 0,664, OR: 0,365-1,208, 95% CI: 0,664).
Kesimpulan: Terdapat peningkatan yang signifikan terkait prevalensi fibrosis miokardium pada tipe non-ischemic, yaitu tipe midwall atau patchy myocardial, dan tipe epicardial pada periode sesudah dan sebelum pandemi COVID-19.

Background: Myocardial Fibrosis (MF) is a disease that occurs due to the cardiac remodeling process of the Extracellular Matrix (ECM) with the appearance of Cardiac Magnetic Resonance Imaging Late Gadolinium Enhancement (CMR--LGE). Etiologically, myocardial fibrosis before the COVID-19 pandemic currently occurs more often due to viral myocarditis caused by adenovirus, hepatitis, and herpes simplex virus (HPV). 1,4 Meanwhile, during the COVID-19 pandemic, viral causes of myocarditis or Myocardial fibrosis are the HPV virus, HIV, Influenza A and B, and the SARS-CoV 2 virus. According to global epidemiological data, it is
17-24%. The American Heart Association (AHA) stated that it was 59.6% with an increase of 12.1% from previous years regarding the incidence of myocarditis. Meanwhile, Basic Health Research (Riskesdas) shows that the prevalence of heart disease is increasing by 1.5% every year, and the prevalence of Myocarditis in Indonesia was 29.2% over the last 10 years.
Objective: This research aimed to determine the comparison of myocardial fibrosis findings on LGE cardiac MRI before and after the COVID-19 pandemic.
Hypothesis: There is a comparison of myocardial fibrosis before and after the COVID-19 pandemic.
Methodology: The study obtained 419 subjects with a prevalence of myocardial fibrosis before the COVID-19 pandemic of 80.4% and 87.6% after the COVID-19 pandemic. A significant comparison was found in the LGE type data with subendocardial ischemic pattern type (p: 0.016, OR: 1.7, 95% CI: 1.102-2.622) and LGE type with epicardial non-ischemic pattern type (p: <0.001, OR: 0.374, 95% CI: 0.242-0.577). The highest prevalence was found in the midwall and patchy myocardial types after the COVID-19 pandemic at 39.7% (p: 0.093, OR: 0.802-17.442, 95% CI: 3.74) compared to the transmural type after the COVID-19 pandemic at 29.9% (p: 0.664, OR: 0.365-1.208, 95% CI: 0.664).
Result: There was a significant increase in the prevalence of myocardial fibrosis in the non-ischemic type, namely the midwall or patchy myocardial type and the epicardial type, before and after the COVID-19 pandemic.
