Abstract
Latar Belakang: Gagal Ginjal Kronis berdampak besar pada kesehatan masyarakat, dengan hemodialisis sebagai salah satu pengobatan utama pada kasus lanjut. Namun, kepatuhan menjalankan hemodialisis masih sering dipengaruhi oleh faktor psikologis sehingga diperlukan studi untuk mengamati hubungannya. Penelitian sebelumnya telah mengidentifikasi hubungan antara faktor psikologis seperti depresi atau kecemasan terhdap kepatuhan hemodialisis dengan menggunakan data yang bersifat subjektif, namun belum diteliti korelasi antara faktor-faktor psikologis tersebut dengan kepatuhan hemodialisis pada pasien Gagal Ginjal Kronis dengan data objektif. Penelitian ini bertujuan untuk mengetahui hubungan korelasi tingkat stress depresi dan kecemasan terhadap kepatuhan hemodialisis pada pasien Gagal Ginjal Kronis.
Metode: Studi cross-sectional dilakukan pada pasien Gagal Ginjal Kronis yang menjalani hemodialisis di Rumah Sakit Umum Siloam. Skala yang tervalidasi, seperti Perceived Stress Scale dan Hospital Anxiety and Depression Scale, digunakan untuk menilai tingkat stres, depresi, dan kecemasan baik dalam bentuk numerik maupun kategorik. Kepatuhan hemodialisis dalam penelitian ini diukur secara objektif menggunakan data sekunder dari rekam medis pasien, secara spesifik dengan menghitung jumlah sesi hemodialisis yang terlewat oleh pasien selama periode penelitian yang ditentukan. Analisa statistic korelasi dan regresi digunakan untuk menguji hubungan tingkat stres, depresi, dan kecemasan dengan kepatuhan hemodialisa.
Hasil: Ditemukan korelasi positif yang signifikan dan keeratan korelasi antara tingkat stres (r = 0.202, p=0.008), depresi (r = 0.179, p=0.02), dan kecemasan (r = 0.211, p=0.006) dengan ketidakpatuhan mengikuti hemodialisis.
Kesimpulan: Tingkat stress, depresi, dan kecemasan mempunyai korelasi positip terhadap kepatuhan hemodialisis pada pasien gagal ginjal kronis di Rumah Sakit Siloam Lippo Karawaci.

Background: Chronic Kidney Disease (CKD) significantly impacts public health, with hemodialysis as a primary treatment for advanced cases. However, adherence to hemodialysis remains challenging for many patients, often influenced by psychological factors. While these factors are known to affect compliance, comprehensive studies investigating their combined effect on adherence, particularly using objective measures such as secondary data within the Indonesia, remain limited. This study examines the relationship between these psychological factors and adherence to hemodialysis among CKD patients.
Methods: A cross-sectional study was conducted on CKD patients undergoing hemodialysis at Siloam General Hospital. Validated scales, including the Perceived Stress Scale (PSS) and the Hospital Anxiety and Depression Scale (HADS), were used to assess levels of stress, depression, and anxiety, measured both numerically and categorically. Hemodialysis adherence was objectively assessed using secondary data from patients' medical records, specifically by counting the number of missed hemodialysis sessions during a defined study period. Adherence outcomes were treated as numerical data (number of missed sessions) rather than categorical, allowing for correlation and regression analyses to examine their relationship with stress, depression, and anxiety levels.
Results: The study included 170 participants. Analysis showed that higher levels of stress (r = 0.202, p = 0.008), depression (r = 0.179, p = 0.020), and anxiety (r = 0.211, p = 0.006) were significantly, though weakly, correlated with increased non-adherence (more missed hemodialysis sessions).
Conclusion: Stress levels, depression, and anxiety have a significant relationship with hemodialysis adherence in patients with chronic kidney failure at Siloam Hospital Lippo Karawaci.
