Abstract

Latar Belakang: Penyakit Paru Obstruktif Kronis (PPOK) adalah penyakit paru yang ditandai dengan adanya keterbatasan aliran udara yang berkelanjutan dan bersifat progresif. PPOK dapat dipicu karena respons inflamasi kronik yang berlebihan pada saluran napas dan jaringan paru akibat terkena gas atau partikel berbahaya sehingga membuat iritasi jaringan paru. Sebagai penyakit kronis, PPOK dapat mempengaruhi kesehatan mental yang kemudian mempengaruhi proses pengobatan, dan berimbas pada derajat keparahan PPOK. Ini membuat sebuah siklus yang saling memperkuat antara PPOK dengan penyakit mental. Namun, sejauh ini belum ada penelitian yang memberikan hasil yang konkrit mengenai hal ini. Oleh karena itu hubungan insomnia dengan depresi terhadap derajat keparahan PPOK perlu dikaji lebih lanjut. Tujuan: Mencari tahu pengaruh insomnia dengan depresi dan efeknya terhadap derajat keparahan PPOK. Metode: Penelitian ini dilakukan dengan metode cross sectional atau potong lintang kepada 74 pasien PPOK pada lokasi sekitar Kabupaten Tangerang tanpa penyakit komorbid dan riwayat penyakit pernafasan lain dari bulan Mei 2023 – Juli 2023. Data diperoleh melalui pengisian keusioner serta wawancara medis. Hasil: Dari 74 orang, 69 orang pasien termasuk kriteria inklusi. Terdapat hubungan signifikan antara insomnia dengan derajat keparahan PPOK (p value = 0,000). Terdapat pula hubungan yang signifikan antara depresi dengan derajat keparahan PPOK (p value = 0,006). Setelah dilakukan uji Multiple Regression Analysis, nilai koefisien regresi pada insomnia sendiri berpengaruh terhadap derajat keparahan PPOK (p value = 0,000), sedangkan depresi secara sendiri tidak berpengaruh pada derajat keparahan PPOK (p value = 0,840). Kesimpulan: Kondisi Kesehatan mental memiliki dampak dalam derajat keparahan PPOK. Namun, apabila insomnia dan depresi terjadi secara bersamaan, insomnia lebih memberi dampak terhadap derajat keparahan PPOK. / Background: Chronic Obstructive Pulmonary Disease (COPD) is a respiratory disease which is marked by a progressive, continuous limitation of airflow. COPD can be triggered by excessive chronic inflammations in the airways and the lung tissues due to exposure of harmful gas or toxic inhalants that cause irritation to the surrounding lung tissue. As a chronic disease, COPD may affect one’s mental health which then may affect the treatment process, thus may then impact the degree of severity of COPD. This creates a mutual reinforcement between COPD and mental illness. However, there has been no research that provides concrete results on this matter. Therefore, the relationship between insomnia and depression on the severity of COPD needed further research. Aim: To investigate the influence of insomnia and depression and its effect on COPD degree of severity. Methods: This study is done with a cross-sectional method on 74 COPD patients around the Tangerang Regency without comorbid diseases and a history of other respiratory diseases from May 2023 to July 2023. Data will be acquired through questionnaires and medical interviews. Results: Of the 74 patients, 69 patients met the inclusion criteria. There is a significant relationship between insomnia and the severity of COPD (p value = 0.000). There is also a significant relationship between depression and the severity of COPD (p value = 0.006). After carrying out the Multiple Regression Analysis test, the regression coefficient value for insomnia influenced the severity of COPD (p value = 0.000), while depression had no effect on the severity of COPD (p value = 0.840). Conclusion: Mental health condition plays a role on the severity of COPD. However, when insomnia and depression occurred simultaneously, insomnia has a greater impact towards the severity of COPD.
