Abstract

Medication error merupakan jenis insiden keselamatan pasien yang paling umum terjadi di dunia, dan menyebabkan kerugian bagi pasien, staf rumah sakit dan biaya perawatan. Untuk mencegah medication error, maka World Health Organisation (WHO) menyusun satu kerangka untuk mengkategorikan informasi yang didapatkan dalam konsep medication safety. Berdasarkan hasil Monthly Medication Error tahun 2017 pelaporan insident report yang terdokumentasi di salah satu Rumah Sakit swasta di Indonesia Bagian Tengah, tercatat 57 peristiwa medication error. Tingginya kasus medication error yang terjadi disebabkan oleh beberapa faktor antara lain faktor manusia, faktor sistem, serta faktor lingkungan. Kajian literatur ini bertujuan untuk mengidentifikasi faktor-faktor yang memengaruhi perawat dalam melaksanakan medication safety di negara berkembang. Lima artikel dipilih berdasarkan kriteria inklusi dan di nilai dengan menggunakan Joanna Briggs Institute critical apprasial. Artikel dianalisa dengan metode simplified approach menggunakan analisa tematik, dengan dua tema dan enam sub tema: faktor internal dan eksternal. Faktor internal, meliputi faktor kondisi tenaga kerja, usia dan pengalaman kerja dan tingkat pengetahuan farmakologi. Faktor eksternal meliputi faktor aturan dan regulasi, supervisi serta faktor lingkungan. Diharapkan bagi instansi-instansi penyedia layanan kesehatan untuk bisa menerapkan medication safety dalam setiap praktek pelayanan = Medication error is a type of patient safety incident that most common in the world, and can cause harm to patients, hospital staff and increase treatment costs. World Health Organisation (WHO) constructed a framework of medication safety concept to prevent medication error. Based on a monthly medication errors report from one private hospitals in Central of Indonesia in 2017, there were 57 incident reports of medication errors that were recorded. The literature review aimed to identify factors that affecting nurses in performing medication safety in developing countries. Five articles were selected based on the inclusion criteria and were apraised using Joanna Briggs Institute Critical Appraisal. The articles were analyzed using simplified approach method, with thematic analysis, with 2 themes and 6 sub-themes: Internal factor and external factor. Internal factor includes health care conditions, age and work experience, and knowledge of pharmacology while external factor includes rule and regulation’s factor, supervision and also environment factor. Health agencies are expected to be able to apply medication safety in all health practice.
