Abstract

Discharge planning is a process of getting patient health care with continuous care for the healing process and in finding the degree of health again. Discharge planning has a goal to make the patient not dependent on the hospital. The discharge planning process is carried out from the beginning of the patient entry until the day of the patient's return. Discharge planning is carried out in the inpatient wards by the implementing nurse by collaborating with other multidisciplinary teams. This study aims to determine the description of discharge planning implementation that is done by nurses at the Siloam Karawaci Public Hospital General Pavilion. This Study is a quantitative research using descriptive development method. The technique of determining the sample by using purposive sampling with the number of respondents 72 patients to be seen from the patient file in the room. The instrument of this research is using document study checklist taken from SOP of Siloam Hospital. Instrument checklist study documentation consists of 16 statements that determine the implementation of discharge planning. Analysis of this research data using univariate analysis. The results showed the implementation of discharge planning in Surgical-Medical inpatient room General Hospital Siloam Karawaci Hospital 29.9%. The highest value of the discharge planning implementation of the form filled by the nurse is 37.5% of them are the date of the patient starting to be treated, the determination of the length of treatment and the place of return of the patient. The lowest value of the discharge planning implementation of the form filled by the nurse of 2.8% is about home care plans / Discharge planning merupakan suatu proses pasien mendapatkan pelayanan kesehatan dengan perawatan yang berkelanjutan untuk proses penyembuhan maupun dalam menemukan derajat kesehatannya kembali. Discharge planning mempunyai tujuan untuk membuat pasien tidak ketergantungan terhadap rumah sakit. Proses pelaksanaan discharge planning dilakukan dari awal pasien masuk sampai pada hari kepulangan pasien. Discharge planning dilaksanakan di ruang rawat inap oleh perawat pelaksana dengan berkolaborasi bersama team multidisiplin lainnya. Penelitian ini bertujuan untuk mengetahui gambaran pelaksanaan discharge planning yang di lakukan oleh perawat di Paviliun Umum Rumah Sakit Siloam Karawaci. Penelitian ini merupakan penelitian kuantitatif dengan menggunakan metode pengembangan deskriptif. Teknik penentuan sampel dengan menggunakan purposive sampling dengan jumlah responden 72 pasien yang akan dilihat dari file pasien di ruangan. Instrumen penelitian ini menggunakan checklist study dokumentasi yang diambil dari SOP Rumah Sakit Siloam. Instrument checklist study dokumentasi terdiri dari 16 pernyataan yang menentukan pelaksanaan discharge planning. Analisis data penelitian ini menggunakan analisa univariat. Hasil penelitian menunjukan pelaksanaan discharge planning di ruang rawat inap Surgical-Medical Paviliun Umum Rumah Sakit Siloam Karawaci berjalan 29,9 %. Nilai tertinggi pelaksanaan discharge planning dari form yang diisi oleh perawat sebesar 37,5 % yaitu, penetapan tanggal pasien mulai dirawat, penetapan lama perawatan dan tempat tujuan kepulangan pasien. Nilai terendah dari pelaksanaan discharge planning dari form yang diisi oleh perawat sebesar 2,8 % yaitu tentang rencan perawatan dirumah.
