Abstract
Medication error is an event that can not only harm the patient but may also endanger the patient's safety done by health care workers especially in the case of patient care (NCCMERP, 2016). In the United States in 2013 there were 251,000 occurrences of medication error causing death, putting medication error into the third leading cause of death in America in that year (CDC, 2013). While at Siloam Hospitals Manado from January to May 1,017 cases occurred. The purpose of this study is to determine the factors that cause errors in drug administration. The research method used is internet search from EBSCO database, and Google Scholar by using the keyword of medication error, factors causing drug error, and factors causes medication error. The result of the analysis showed that the most medication error occurred in the prescribing phase of 98.69%, then the drug delivery phase was 63%, and the smallest in the compounding phase was 4.9%. Factors contributing to medication error include: communication (eg communication, communication failure), environmental conditions, interruptions at work, workload, and staff education. This research is expected to be information as input in creating prevention strategies and minimize the occurrence of medication error / Kesalahan pemberian obat (Medication error) adalah suatu kejadian yang tidak hanya dapat merugikan pasien tetapi juga dapat membahayakan keselamatan pasien yang dilakukan oleh petugas kesehatan khususnya dalam hal pelayanan pengobatan pada pasien (NCCMERP, 2016). Di Amerika Serikat pada tahun 2013 tercatat 251.000 kejadian medication error yang menyebabkan kematian, hal ini menempatkan medication error menjadi penyebab kematian ketiga di Amerika pada tahun tersebut (CDC, 2013). Sedangkan di Siloam Hospitals Manado dari bulan januari sampai bulan Mei terhitung 1.017 kasus yang terjadi. Tujuan dari penelitian ini adalah untuk mengetahui faktor-faktor yang menyebabkan terjadinya kesalahan dalam pemberian obat. Metode penelitian yang digunakan ialah dengan penelusuran internet dari database EBSCO, dan Google Scholar dengan menggunakan kata kunci kesalahan pemberian obat, medication error, faktor-faktor penyebab kesalahan pemberian obat, dan factors causes medication error. Hasil analisis menunjukan kejadian medication error terbanyak ada pada fase peresepan sebesar 98,69%, kemudian fase pemberian obat sebesar 63%, dan paling kecil pada fase peracikan sebesar 4,9%. Faktor-faktor yang berkontribusi pada medication error antara lain: komunikasi (mis-komunikasi, kegagalan dalam berkomunikasi), kondisi lingkungan, gangguan (interupsi) pada saat bekerja, beban kerja, dan edukasi staf. Penelitian ini diharapkan dapat menjadi informasi sebagai masukan dalam menciptakan strategi pencegahan dan meminimalkan terjadinya medication error.
