Abstract

Diabetes mellitus (DM) is a chronic disease characterized by an inability of the body to metabolism the carbohydrates, fats which caused hyperglycemia (increased blood sugar levels). According to the WHO (2016), diabetes mellitus disease is the sixth death caused in the world. Around 1.3 million people death by diabetes and 4% between died before the age of 70 years old. In 2030, DM is estimated as the 7th death caused in the world. While in Indonesia, DM patient amount 21,3 million people and the 7th that has the highest prevalence of diabetes. The purpose of this research is to know the description of the implementation discharge planning by a nurse on the patients diabetes mellitus type 2 in Siloam Hospitals Makassar. The method of research is quantitative descriptive. The population in this research is the nurse of inpatient care in Siloam Hospitals Makassar as much 105 nurse. The sample needs 50 people using minimal sample size. The results of the research showed that the respondents who did the discharge planning to diabetes mellitus type 2 patients are categorized good as many as 37 people (74%), while quite as many as 8 people (16%), and less as many as 5 people (10%). Suggestions for further research to further and in-depth research on the discharge planning in patient diabetes mellitus type 2 in hospitals such as factors that affect nurse knowledge about the implementation of discharge planning in patien diabetes mellitus type 2 / Diabetes Mellitus (DM) merupakan suatu penyakit kronis yang ditandai oleh ketidakmampuan tubuh untuk melakukan metabolisme karbohidrat, lemak dan protein sehingga menyebabkan hiperglikemia (peningkatan kadar gula darah). Menurut WHO (2016), penyakit diabetes mellitus merupakan penyebab kematian keeman di seluruh dunia. Sekitar 1,3 juta orang meninggal akibat diabetes dan 4 % diantaranya meninggal sebelum usia 70 tahun. Pada tahun 2030, diperkirakan DM menempati urutan ke-7 penyebab kematian di dunia. Sedangkan di negara Indonesia diperkirakan akan memiliki penderita DM sebanyak 21,3 juta jiwa dengan urutan ke-7 yang memiliki prevalensi diabetes tertinggi. Tujuan penelitian ini mengetahui gambaran pelaksanaan perencanaan pulang oleh perawat pada pasien diabetes mellitus tipe 2 di Siloam Hospitals Makassar. Metode penelitian ini adalah desain deskriptif kuantitatif. Populasi dalam penelitian ini adalah perawat rawat inap di Siloam Hospitals Makassar sebanyak 105 perawat. Sampel yang diperlukan sebesar 50 orang menggunakan perhitungan minimal sampel size. Hasil penelitian menunjukkan bahwa responden yang melakukan perencanaan pulang pada pasien diabetes mellitus tipe 2 dikategorikan baik yaitu sebanyak 37 orang (74%), sedangkan cukup sebanyak 8 orang (16%), dan kurang sebanyak 5 orang (10%). Saran agar peneliti selanjutnya dapat melakukan penelitian lebih lanjut dan mendalam mengenai pelaksanaan perencanaan pulang pada pasien diabetes mellitus tipe 2 di rumah sakit seperti faktor-faktor yang memperngaruhi pengetahuan perawat mengenai pelaksanaan perencanaan pulang pada pasien diabetes mellitus tipe 2.
