Medication errors are an undesirable event and should be avoided. Sometimes medication errors are ignored, but medication errors can be avoided in order not to adversely affect patients or health care providers. Medication errors are divided into four parts: prescribing errors, transcribing errors, dispensing errors and administration errors. The purpose of this study was to identify the description of the incidence of medication errors, especially about the incidence rate and the type of medication errors. The research method used in this research is descriptive quantitative method. The sampling technique used was purposive sampling technique with total 127 samples in the form of incident report document. The results of the study were 8% or 10 prescribing errors, 14% or 18 transcribing errors, 59% or 75 incidents of dispensing errors and 19% or 24 events of administration errors. Suggestions for the next researcher is to do research on what factors can cause medication errors and examine what things can be done to change or improve the circumstances and the incidence of medication errors / Medication errors adalah suatu kejadian yang tidak diinginkan dan harusnya dapat dihindari. Seringkali medication errors tidak diperhatikan atau diabaikan, namun sebenarnya medication errors dapat dihindari agar tidak memberikan dampak buruk bagi pasien maupun bagi penyedia layanan kesehatan seperti rumah sakit. Medication errors dibagi menjadi 4 bagian, yaitu prescribing errors, transcribing errors, dispensing errors dan administration errors. Tujuan penelitian ini adalah untuk mengidentifikasi gambaran kejadian medication errors khususnya tentang angka kejadian dan jenis medication errors. Metode penelitian yang digunakan pada penelitian ini adalah metode kuantitatif deskriptif. Teknik pengambilan sampel yang digunakan adalah teknik purposive sampling dengan total 127 sampel berupa dokumen incident report. Hasil penelitian yang didapatkan adalah 8% atau sebanyak 10 kejadian prescribing errors, 14% atau 18 kejadian transcribing errors, 59% atau 75 kejadian dispensing errors dan 19% atau 24 kejadian administration errors. Saran bagi peneliti selanjutnya adalah dilakukannya penelitian tentang faktor apa saja yang dapat menyebabkan medication errors dan meneliti hal apa saja yang dapat dilakukan untuk mengubah atau memperbaiki keadaan dan angka kejadian medication errors.
