The accuracy of patient identification is very important because it is one of the requirements in the Hospital Accreditation Standard. Proper identification can reduce the risk of errors such as sampling, the process of giving medicine to the patient, as well as proof that the patient’s and family’s rights have been fulfilled in receiving safe and quality services. This study was conducted to determine the system of patient identification and to analyze the nurse’s knowledge level in the implementation of patient identification before performing the nursing action in Out patient Department Siloam Hospitas Kebon Jeruk. The method of this study is descriptive quantitative. The population of this study was 45 nurses who were working in outpatient department. The technique sampling was total sampling. Data collection was conducted by distributing questionnaires to nurses to know the necessary data about how the quality of health service in Out patient Department Siloam Hospitals Kebon Jeruk. The result of this study showed that the respondents who have high knowledge were 29 nurses (64.50%), the respondents who have medium knowledge were 14 nurses (31.10%), and the low knowledge were 2 nurses (4.40%) / Ketepatan identifikasi pasien sangat penting karena merupakan salah satu persyaratan yang terdapat dalam Standar Akreditasi Rumah Sakit. Identifikasi yang tepat dapat mengurangi risiko terjadinya kesalahan seperti contoh proses pemberian obat kepada pasien, serta menjadi bukti bahwa hak pasien maupun keluarga sudah terpenuhi dalam menerima pelayanan yang aman dan bermutu. Penelitian ini dilakukan untuk mengetahui sistem identifikasi pasien yang digunakan dan menganalisa tingkat pengetahuan perawat dalam pelaksanaan identifikasi pasien di ruang rawat jalan Siloam Hospitals Kebon Jeruk. Metode penelitian ini adalah deskriptif kuantitatif. Populasi dari penelitan ini adalah 45 perawat, teknik sampling yang digunakan adalah total sampling. Pengumpulan data dilakukan dengan membagikan kuesioner kepada perawat untuk memperoleh data-data yang diperlukan tentang bagaimana kualitas pelayanan kesehatan di ruang rawat jalan Siloam Hospitals Kebon Jeruk. Subjek dalam penelitian ini adalah perawat di seluruh ruang rawat jalan Siloam Hospitals Kebon Jeruk. Hasil penelitian menunjukkan responden yang memiliki pengetahuan tinggi sebanyak 29 perawat (64.50 %), tingkat pengetahuan sedang sebanyak 14 perawat (31.10%) dan yang memiliki pengetahuan rendah sebanyak 2 perawat (4.4 %).
