Abstract

ABSTRAK Valencia Angel Soeryanto (01071180076) HUBUNGAN GANGGUAN KECEMASAN DENGAN KUALITAS HIDUP PADA PASIEN LUPUS ERITEMATOSUS SISTEMIK DI INDONESIA. Latar belakang: Lupus Eritematosus Sistemik (LES) adalah penyakit inflamasi autoimun kronis yang memiliki gambaran klinis yang sangat luas serta memiliki tampilan perjalanan penyakit yang beragam serta dapat mempengaruhi banyak organ di tubuh. Salah satu manifestasi yang dapat terjadi adalah gangguan neuropsikiatri dimana yang paling sering adalah gangguan kecemasan. Kecemasan pada pasien LES juga dapat ditimbulkan dari konsumsi kortikosteroid harian karena mengganggu neurotransmitter dan merusak blood brain barrier. Kualitas hidup dipengaruhi secara tidak langsung oleh banyak faktor seperti tingkat pendidikan, status sosial ekonomi, gangguan tidur dan aktivitas penyakit melalui manifestasi kecemasan. Sudah ada penelitian mengenai hubungan gangguan kecemasan dan kualitas hidup pada pasien LES, namun penelitian hanya menggunakan responden orang bandung sehingga penelitian ini mengambil responden dari latar belakang wilayah yang lebih luas. Tujuan: Mengetahui hubungan dari gangguan kecemasan dengan kualitas hidup pada pasien LES wanita di Indonesia. Metode: Penelitian ini menggunakan metode potong lintang (cross sectional). Jumlah sampel sebanyak 90 orang, yang merupakan penderita LES berjenis kelamin wanita yang dipilih menggunakan teknik purposive sampling. Pengambilan data dilakukan pada periode Januari-Februari 2021. Hasil penelitian diolah dengan program SPSS 24,0, uji korelasi bivariat dengan uji T tidak berpasangan, Mann-whitney, Spearman dan One Way anova serta analisis multivariat regresi linear. Hasil: Dari 90 total sampel yang terkumpul, ditemukan bahwa gangguan kecemasan(p=0,001), gangguan tidur(p<0,001), aktivitas penyakit(p=0,001), dosis kortikosteroid harian( p<0,001) dan tingkat pendidikan(p=0,016) memiliki hubungan yang signifikan terhadap kualitas hidup. Hubungan tidak signifikan antara status pernikahan(p=0,41), IMT(p=0,377), pendapatan(p=0,29), aktivitas fisik(p=0,05), penyakit kronis (p=0,47) dan usia (p=0,18) terhadap kualitas hidup. Kesimpulan: Ditemukan adanya hubungan yang signifikan antara gangguan kecemasan terhadap kualitas hidup pada pasien LES di Indonesia. Background: Systemic Lupus Erythematosus (SLE) is a chronic autoimmune inflammatory disease that has a very broad clinical picture and a diverse disease course and also can affect many organs in the body. One of the manifestations that can occur is neuropsychiatric disorders, where the most common is the anxiety disorders. Anxiety in SLE patients can also arise from daily consumption of corticosteroids because they able to interfere neurotransmitters and damage the blood brain barrier. Quality of life is influenced indirectly by many factors such as education level, socioeconomic status, sleep disorders, and disease activity through the manifestations of anxiety. There is a research that have already discussed the relationship between anxiety disorders and quality of life in SLE patients, but the study is specifically for Bandung-based respondents. Hence, this study took respondents from a broader area background. Objectives: To know the relationship between anxiety disorders and quality of life in female SLE patients in Indonesia. Methods: This study used a cross sectional method. The number of samples was 90 women with SLE who were selected using purposive sampling technique. Data were collected in the period of January-February 2021. The results of the study were processed with the SPSS 24.0 program, bivariate correlation test with unpaired T test, Mann-Whitney, Spearman, One Way ANOVA, and multivariate linear regression analysis. Results: Out of the 90 total samples collected, it was found that anxiety disorders (p = 0.001), sleep disorders (p <0.001), disease activity (p = 0.001), daily corticosteroid dose (p <0.001) and education level (p = 0.016) has a significant relationship towards the quality of life. There was no significant relationship between marital status (p = 0.41), BMI (p = 0.377), income (p = 0.29), physical activity (p = 0.05), chronic disease (p = 0.47) and age (p = 0.18) against the quality of life. Conclusion: There was a significant relationship between anxiety disorders with quality of life on SLE patients in Indonesia.
