Abstract
CV Dwi Jaya Abadi merupakan salah satu home industri kulit yang memproduksi tas dan koper dengan sistem make to order dan make to stock. Dalam menjalankan proses produksinya, CV Dwi Jaya Abadi seringkali menghadapi risiko dalam bentuk kerusakan atau gangguan selama proses produksi tas dan koper berlangsung. Oleh karena itu, sebagai upaya meningkatkan kualitas produk perlu dilakukan pengawasan dan pengendalian untuk mengurangi kemungkinan terjadinya risiko kegagalan tersebut yang dapat berdampak pada kualitas produk. Failure Mode and Effect Analysis (FMEA) merupakan metode yang digunakan dalam penelitian ini untuk mengidentifikasi risiko kegagalan pada proses produksi. Proses identifikasi dan penilaian risiko diperoleh dari hasil wawancara dan penyebaran kuesioner kepada enam (6) orang karyawan perusahaan yang memahami tentang proses produksi secara keseluruhan. Penilaian risiko dengan FMEA diukur melalui tiga kategori yakni severity, occurrence, dan detection. Hasil identifikasi dan penilaian risiko menunjukkan bahwa terdapat 14 risiko yang berpotensi mengganggu kualitas produk. Dari 14 risiko tersebut melalui hasil pemetaan risiko (probability-impact matrix), dapat diketahui risiko yang tergolong kritis dan harus dimitigasi. Risiko-risiko kritis tersebut antara lain; pola rumit atau tidak jelas, potongan bahan tidak sesuai ukuran atau spesifikasi, hasil sesetan terlalu tipis bahkan robek, pengeleman kurang kuat dan kotor, jahitan kurang rapat dan mudah terbuka kembali, dan aksesoris tidak terpasang dengan baik dan mudah lepas. Selanjutnya usulan mitigasi risiko dilakukan dengan menggunakan metode Poka Yoke. / CV Dwi Jaya Abadi is one of leather home industry that produced bags and suitcases with make to order and make to stock systems. In its production process, CV Dwi Jaya Abadi often face the various risks in the form damage or disruption during the production process. Therefore, as an effort to improve the product quality, needed supervisor and controlling during production process to minimize probability of quality risk occurrence. Failure Mode and Effect Analysis (FMEA) is a method that used in this research to identify the risk of failure in the production process. Identification and risk assessment process obtained from interview and questionnaires to the six (6) employees of the company who understand the whole production process. Risk assessment in FMEA is measured by three categories like severity, occurrence, and detection. Risk identification and risk assessment of FMEA results indicated that there are 14 quality risks in bags and suitcases production process. Through probability-impact matrix, can be seen that there are some critical quality risk and must be mitigated. Critical risks include, the risk of complicated and unclear pattern, cutting of material does not fit with the size or specification, the result of skiving materials too thin, dirty gluing and less powerfull, less dense of sewing materials and easy to open, accesories are not installed properly and easily separated. Suggestions of risk mitigation were solved using Poka Yoke method.
