Abstract

Background: In 2017, the World Health Organization (WHO) put out a list of pathogens that desperately need new antibiotics. Acinetobacter baumannii ranked first on that list due to its resistance to various types of antibiotics, including carbapenems, which are thought to be a last-resort antibiotic. Due to its MDR character, infection with A. baumannii can significantly increase the morbidity and mortality of patients. Thus, a study on its phenotype can greatly benefit the community for both the prevention and treatment of A. baumannii infections. Aim: The aim of this study is to characterize the phenotypes of A. baumannii in patients of Rumah Sakit Siloam Lippo Village and its relationship with the gender, age, and types of sample of patients. Method: This study is a retrospective unpaired analytic comparative categoric with a cross-sectional study design. The data that will be used in this research will be obtained from the Siloam Hospital laboratory database during a time period of January 2016 to December 2017. Data tabulation will use Microsoft Excel 2016 while data analysis will use SPSS 23. Statistical analysis is done using likelihood ratio and continuity correction. Results: This research found that MDR A. baumannii is more prevalent in men, people aged between 45-64 years old, and in samples whose isolate is derived from the sputum. It was also found that gender had no significant relationship with any A. baumannii resistance phenotypes. But age groups has a significant relationship with the resistance phenotype Gen Net Ami Tob, Gen Net Ami, and Gen Tob Ami. And the source of isolate has a significant relationship with the resistance phenotype Gen Net, Gen, Tob Gen, Net, Tob Net, and all the phenotypes that contributes to resistance towards beta lactam antibiotics / Latar Belakang: Pada tahun 2017, The World Health Organization (WHO) mengeluarkan sebuah daftar patogen yang sangat memerlukan antibiotik baru dikarenakan resistensinya pada antibiotik-antibiotik yang telah ada. Acinetobacter baumannii menduduki peringkat pertama karena multidrug resistant (MDR) A. baumannii telah menjadi resisten pada berbagai jenis antibiotik, termasuk antibiotik lini terakhir yaitu karbapenem. Infeksi oleh MDR A. baumannii sangat sering ditemui pada pasien rumah sakit, dan hal ini meningkatkan morbiditas dan mortalitas pasien. Oleh karena itu, pengertian akan fenotipe A. baumannii berguna untuk mencegah dan mengobati infeksi A. baumannii. Tujuan Penelitian: Tujuan dari penelitian ini adalah untuk melakukan karakterisasi fenotipe MDR A. baumannii pada pasien Rumah Sakit Siloam dan untuk melihat hubungannya dengan jenis kelamin, usia, dan jenis sampel pasien. Metode: Penelitian adalah studi retrospektif analitik komparatif kategorik tidak berpasangan dengan desain potong lintang. Data yang akan digunakan adalah data dari database laboratorium Paviliun A dan Paviliun B Rumah Sakit Siloam Lippo Village pada periode waktu Januari 2016 hingga Desember 2017. Uji statistik yang akan digunakan adalah uji statistik lihelihood ratio dan continuity correction. Hasil: Penelitian ini menemukan bahwa MDR A. baumannii lebih banyak ditemukan pada pasien pria dibanding wanita, pada kelompok usia 45-64 tahun, dan pada sampel yang sumber isolatnya adalah sputum. Ditemukan juga bahwa jenis kelamin tidak memiliki hubungan yang signifikan dengan fenotipe resistensi A. baumannii, sementara kelompok usia memiliki hubungan yang signifikan dengan fenotipe resistensi Gen Net Ami Tob, Gen Net Ami, dan Gen Tob Ami, dan sumber isolat memiliki hubungan yang signifikan pada fenotipe resistensi Gen Net, Gen, Tob Gen, Net, Tob Net, dan semua fenotipe resistensi pada antibiotik golongan beta laktam.
