Abstract
Latar belakang: Data Riskesdas 2018 menyatakan bahwa prevalensi Penyakit Jantung Koroner di Indonesia berjumlah 15 dari 1000 orang. Coronary Artery Disease (CAD) dapat menyebabkan penyempitan obstruktif atau non-obstruktif pada arteri koroner. Salah satu manifestasi klinis CAD ialah iskemia miokardia, yang dapat menghasilkan keluhan angina tipikal, atipikal, dispnea, dan tanpa gejala. Pada beberapa studi ditemukan perbedaan presentasi keluhan iskemia miokardia antara pria dan wanita. Tujuan: Untuk mengetahui apakah terdapat hubungan antara jenis kelamin dengan karakteristik keluhan iskemia miokardia pada pasien Obstructive Coronary Artery Disease. Metode: Penelitian ini menggunakan studi observasional analitik komparatif kategorik tidak berpasangan dengan metode potong lintang yang termasuk studi kualitatif dan dilakukan bulan April-Juni 2024 pada pasien Obstructive Coronary Artery Disease berusia 20-80 tahun. Penelitian ini dilakukan dengan wawancara dan rekam medis. Data diolah menggunakan program Statistical Package for the Social Sciences (SPSS) 29 yang diuji secara statistik melalui metode chi-square. Hasil: Penelitian ini menghasilkan hubungan yang signifikan antara jenis kelamin dengan karakteristik keluhan iskemia miokardia pada pasien Obstructive Coronary Artery Disease. Prevalensi jenis kelamin didapatkan sampel pria sebesar 72.8% dan sampel wanita sebesar 27.2%. Pada pria diperoleh sebanyak 52% sampel angina tipikal dan 48% sampel bukan angina tipikal. Pada wanita diperoleh sebanyak 26.8% sampel angina tipikal dan 73.2% sampel bukan angina tipikal. Kesimpulan: Jenis kelamin memiliki hubungan yang signifikan dengan karakteristik keluhan iskemia miokardia pada pasien Obstructive Coronary Artery Disease. Pria lebih cenderung menunjukkan gejala angina tipikal, sedangkan wanita lebih sering mengalami bukan angina tipikal. / Background: Data from Riskesdas 2018 states that the prevalence of Coronary Heart Disease in Indonesia is 15 per 1000 people. Coronary Artery Disease can result in obstructive or non-obstructive stenosis of coronary arteries. One of the clinical manifestations is myocardial ischemia, which can cause typical angina, atypical angina, dyspnea, and asymptomatic cases. Several studies have found differences in the presentation of myocardial ischemia symptoms between gender. Objective: To determine whether there is a relationship between gender and characteristics of myocardial ischemia symptoms in patients with Obstructive Coronary Artery Disease. Method: This research use unpaired categorical comparative analytical observational study with cross-sectional design and was carried out April-June 2024 on patients with obstructive coronary artery disease aged 20-80 years. The study was conducted using interviews and medical record. Data was processed using the Statistical Package for the Social Sciences (SPSS) 29, and statistical testing was performed using the chi-square method. Result: This research found a significant relationship between gender and the characteristics of myocardial ischemia symptoms in patients with Obstructive Coronary Artery Disease (p=0.002). Gender prevalence showed that 72.8% were male and 27.2% were female. In men, 52% samples had typical angina and 48% had non- typical angina. In women, 26.8% samples had typical angina and 73.2% had non- typical angina. Conclusion: Gender has a significant relationship with characteristics of myocardial ischemia symptoms in patients with Obstructive Coronary Artery Disease. Men are more likely to present with typical angina, while women exhibit more non-typical angina symptoms.
