Abstract

Background HCAI often occur on ICU patients, especially by gram-negative bacteria. There are many factors that could increase the risk of HCAI, one of which is prophylactic antifungal use. ICU patients with certain comorbids often got treated with prophylactic antifungal, thus raising concern on the relation between these two variables. Although there have been many in vitro and animal researches on bacterial-fungal interactions, only few directly research effect of prophylactic antifungal use on gram-negative bacteria infection clinically. Hence, more evidence is required clinically on this topic. Aim To find the effect of prophylactic antifungal use on gram-negative bacteria infection in ICU patients. Material and Methods A retrospective observational study is done using cross-sectional design on 261 target samples from ICU patients of Siloam Teaching Hospital from May 2016 to December 2017. Purposive sampling technique was used. Results Out of 261 samples, 147 (56,3%) was given antifungal therapy. Antifungal therapy was found to be associated with gram-negative bacterial infections (OR: 2,941, 95% CI: 1,585-5,457, p-value: 0,001), from which the most significant one is with echinocandins therapy (OR: 6,662, 95% CI: 3,250-13,495, p-value: <0,001). Conclusion This study shows that antifungal therapy is associated with gram-negative bacteria infection. Further researches are needed to confirm the results as there is still few researches on this topic / Latar Belakang HCAI sering terjadi pada pasien ICU, terutama oleh bakteri gram-negatif. Banyak faktor dapat meningkatkan risiko HCAI, salah satunya penggunaan profilaksis antijamur. Pasien ICU dengan komorbid tertentu sering diberikan profilaksis antijamur, sehingga perlu dilakukan penelitian mengenai hubungan kedua hal ini. Banyak penelitian sudah dilakukan untuk mengetahui interaksi jamur-bakteri secara in vitro dan pada binatang, namun masih sedikit yang secara langsung meneliti pengaruh penggunaan profilaksis antijamur terhadap infeksi bakteri secara klinis. Dengan demikian, diperlukan bukti lebih lanjut secara klinis mengenai topik ini. Tujuan Untuk mengetahui pengaruh penggunaan profilaksis antijamur terhadap kejadian infeksi bakteri gram-negatif pada pasien ICU. Metode Penelitian Penelitian dilakukan secara observasional retrospektif dengan desain studi potong lintang. Sampel penelitian sebanyak 261 subyek dari pasien ICU Rumah Sakit Pendidikan Siloam dari bulan Mei 2016 hingga Desember 2017. Pengambilan sampel dilakukan secara purposive sampling. Hasil Didapatkan 261 sampel dimana 147 (56,3%) diberikan terapi antijamur. Ditemukan pemberian terapi antijamur memiliki hubungan dengan infeksi bakteri gram-negatif (OR: 2,941, 95% CI: 1,585-5,457, p-value: 0,001). Hasil paling signifikan ditemukan pada terapi dengan golongan echinocandin (OR: 6,662, 95% CI: 3,250- 13,495, p-value: <0,001). Kesimpulan Hasil studi ini menunjukkan bahwa terapi antijamur memiliki hubungan dengan infeksi bakteri gram-negatif. Namun masih dibutuhkan studi-studi berikutnya untuk mendapatkan lebih banyak bukti karena masih sedikit penelitian pada topik ini.
