Abstract

Appendectomy and Anesthesia could evoke stress respons, that increase blood glucose concentration and make a hyperglycemia condition especially in patient with diabetes mellitus type 2. It is important to find a safe anesthesia procedure for patient especially with diabetes mellitus type 2, whether general anesthesia or spinal anesthesia to minimize the increase of blood glucose concentration. To decide the increase of blood glucose concentration that affected by general anesthesia compare to spinal anesthesia in patient with diabetes mellitus type 2 before and after appendectomy in Siloam Hospital Lippo Village. This study used cross sectional design. Targeted sample of research are 38 subjects from patients who has DM Type 2 and underwent appendectomy surgery with general or spinal anesthesia at Siloam Hospital Lippo Village. Secondary data is taken from the patients’ medical records. The data obtained tabulated using Microsoft Excel 2007 and analyzed using SPSS 23 program, with Mann-U Whitney diagnostic test. There was no significant difference in increase of blood glucose concentration using general or spinal anesthesia in patients with DM Type 2 post appendectomy surgery (p>0.05). The blood glucose increase in both patients DM Type 2 with general and spinal anesthesia. From this study, there was no significant difference in increase of blood glucose concentration in patients with DM Type 2 given general anesthesia or spinal anesthesia post appendectomy surgery in Siloam Hospital Lippo Village / Operasi apendiktomi dan anestesi dapat menyebabkan timbulnya respon stress dari tubuh yang akibatnya dapat menaikan kadar gula darah. Kenaikan kadar gula darah pasca operasi akan lebih besar pada pasien dengan diabetes mellitus tipe 2. Oleh karena itu perlu untuk dicari cara anestesi yang paling sedikit menaikkan kadar gula darah terutama pada pasien dengan diabetes mellitus tipe 2, dengan menggunakan anestesi umum ataukah anestesi spinal. Untuk menentukan adanya pengaruh perbedaan pembiusan (umum atau spinal) terhadap peningkatan kadar gula darah pada pasien DM tipe 2 pasca operasi apendiktomi di Rumah Sakit Siloam Lippo Village Penelitian ini menggunakan desain studi potong lintang. Sampel sebanyak 38 subjek dari pasien yang memiliki DM tipe 2 dan menjalani operasi apendiktomi dengan anestesi umum atau anestesi spinal di Rumah Sakit Umum Siloam Lippo Village. Data sekunder diambil dari rekam medis pasien. Data yang didapatkan ditabulasi menggunakan Microsoft Excel 2007 dan dianalisis menggunakan program SPSS 23, dengan uji diagnostic berupa Mann-U Whitney. Tidak terdapat perbedaan secara signifikan antara pemakaian anestesi umum dengan anestesi spinal pada kenaikan darah pasien DM Tipe 2 pasca operasi apendiktomi (p>0.05). Terdapat peningkatan kadar gula darah pada pasien DM Tipe 2 pasca operasi apendiktomi yang diberikan anestesi umum maupun anestesi spinal. Dari penelitian ini, tidak terdapat perbedaan secara signifikan pada kenaikan kadar gula darah yang diberikan pada pasien DM Tipe 2 dengan anestesi umum dibandingkan dengan anestesi spinal pasca apendiktomi di Rumah Sakit Umum Siloam Lippo Village.
